Additional fields appear on the Title X 2.0 content for the practices that need to adhere to the Title X FPAR 2.0 standards.

Document the Birth Control Method Details
You can document the birth control method in the SOAP template. Open NextGen® Enterprise EHR. Open the SOAP template. On the Assessment/Plan panel, enter the birth-control method details in Birth control method (BCM) at end of visit. Note: When the master document is generated for the visit, the birth-control method appears in the document. Parent topic: Document Assessments and Plans

Add Birth Control Information
Ensure that Title X fields are configured in the Practice Configuration template. Open NextGen® Enterprise EHR. Select a patient, create an encounter and then select the Specialty as Family Practice and Visit Type as Office Visit. On the SOAP template, expand the Assessment / Plan panel. To include the confidential details in the master documents, select the Include all Title X contraception information in the document with patient permission checkbox. To add a reason for not mentioning Birth Control Information (BCM), click Reason for No Birth Control Method at End of Visit. To add a contraception method, click Contraception Provision Method. To add or update BCM, click Birth Control Information at End of Visit. Note: You can add up to two birth control methods. Save the changes. Parent topic: Configure Title X in Assessment / Plan

Document Assessments and Plans
Open NextGen® Enterprise EHR. Open the SOAP template, and then expand the Assessment/Plan panel. To add the patient's assessment and plan, select an appropriate link from the left navigation bar, or select Add/Update. To copy assessments from a previous encounter to the current encounter, select Dx/AP History.If you have copied and edited information from a previous encounter, a label appears in the Assessment/Plan panel and applicable documents indicating that the information is copied and edited. Note: You can configure the functionality to copy assessments from previous encounters in the Copy Previous Visit Information section of the Practice Configuration template. Without the configuration, the Assessment, Diagnosis Copy Forward window opens in read-only mode when you select Dx/AP History. Configure the Title X Fields for Practices
To document the Title X report 2.0 details, you can configure the Title X Fields option on the Practice Configuration template . Open NextGen® Enterprise EHR. From the File menu, select System \ Practice Template. The Select Template window opens. From the Show section, select All. Click the Practice tab. A list of practice level templates appears. Select Practice Configuration, and then click OK. The Practice Configuration template opens. From the Title X Fields section, select the Enable Title X fields check box. Configuring Title X Fields
You can configure Title X Fields to include all contraception information in the document with patient permission. The Title X report 2.0 is generated by all family planning clinics receiving Title X grants from the government. Enable Title X fields in practice template If you select the Enable Title X fields checkbox, additional fields appear on the Title X 2.0 content for the practices that need to adhere to the Title X FPAR 2.0 standards. For example, when you configure the Title X Fields setting on the Practice Configuration template, the Family planning encounter checkbox appears in the General panel on the Intake template and the birth control method information appears in the Assessment/Plan panel on the SOAP template. Update Patient Title X Information
Open NextGen® Enterprise EHR. Select a patient and create an encounter. To open the Templates module on a patient encounter, do one of the following: On the Categories or Encounter History tab, select Open Template. On Top Toolbar, select Templates. On the main menu, select File > New > Template. The Select Template window opens. In the Show section, select All. Select Title X 2.0, and then click OK. The Title X 2.0 template opens. Title X Template Review the patient’s Title X 2.0 details. To qualify the encounter to be part of Title X / FPAR 2.0 report, you can select the Family planning encounter check box.
