Update Patient Title X Information

Select a patient and create an encounter.
To open the Templates module on a patient encounter, do one of the following:
On the
Categories
or
Encounter History
tab, select
Open Template
.

Top Toolbar

On the main menu, select

New

.
The
Select Template
window opens.
In the
Show
section, select
All
.
Select
Title X 2.0
, and then click
OK
.
The
Title X 2.0
template opens.
Review the patient’s Title X 2.0 details.
To qualify the encounter to be part of Title X / FPAR 2.0 report, you can select the
Family planning encounter
check box.
To add or update any details, click the appropriate link in each panel and then update the details.

Configure the Title X Fields for Practices
To document the Title X report 2.0 details, you can configure the Title X Fields option on the Practice Configuration template . Open NextGen® Enterprise EHR. From the File menu, select System \ Practice Template. The Select Template window opens. From the Show section, select All. Click the Practice tab. A list of practice level templates appears. Select Practice Configuration, and then click OK. The Practice Configuration template opens. From the Title X Fields section, select the Enable Title X fields check box. General Details
On the Title X 2.0 template, the General panel displays a patient’s general information. Title X General panel This table explains the fields in the General panel: Field Description First The patient's first name is retrieved from the Patient Demographic template. Last The patient's last name is retrieved from the Patient Demographic template. Middle The patient's middle name is retrieved from the Patient Demographic template. Encounter date Displays the patient’s current or latest encounter date. Family planning encounter Select the Family planning encounter check box to include the current encounter details in the FPAR 2.0 report. Contraceptive counseling This finding is retrieved from the My Plan template. You can also select the check box to update. Title X 2.0 Template Overview
The Title X 2.0 template displays all the Title X 2.0 clinical information recorded for a patient during a family planning encounter. You can review the present and previously recorded patient details for Family Planning Annual Report (FPAR) 2.0. The patient’s recorded Title X 2.0 information is categorized into various panels, such as General, Social History, Confidential History, Assessment / Plan Vital Signs, and Testing and Results. The Title X FPAR 2.0 report is required under the Title X Service Grants and is generated by all family planning clinics receiving the Title X grants. The Office of Population Affairs (OPA) requires all Title X grantees to collect and submit FPAR data at the encounter level for the yearly reporting period, starting January 1 through December 31. The Title X FPAR report 2.0 must be set up and licensed for your practice. When the Title X 2.0 software is installed, the lab tests and the results also appear on the Title X 2.0 template. View Lab Orders
Open NextGen® Enterprise EHR. Select a patient and create an encounter. To open the Templates module on a patient encounter, do one of the following: On the Categories or Encounter History tab, select Open Template. On Top Toolbar, select Templates. On the main menu, select File > New > Template. The Select Template window opens. From the Select All template, open Title X 2.0 template. In the Testing and Results panel, select the order type. The available orders for the selected order type appear in the grid. Select All Orders. The details of each order appear in the grid. Configure Title X in Assessment / Plan
Open NextGen® Enterprise EHR. From the File menu, select System \ Practice Template. The Select Template window opens. From the Show section, select All. Select the Practice tab.A list of practice level templates appears. Select Practice Configuration, and then select OK.The Practice Configuration template opens. Open a Template
In NextGen Care® Population Management Hub, to view the patients list, do one of the following: From the Cohorts tab, run a report. From the My Lists tab, run a list. From the Appointment Book tab, filter the patients. The patient list appears in the patient grid. In the patient grid, select the check box to the left of the patient's name. From Chart, select EHR Chart. A list appears displaying up to ten favorite templates. Do one of the following: To open a template with an existing unlocked encounter, select an encounter from the required template. To open a template with a new encounter, select the template name, and then select Yes to create a new encounter. Note: For NextGen® Enterprise EHR version 5.9 or earlier, you cannot select an encounter from NextGen Care® Population Management Hub. If an unlocked encounter exists for the current date, the template opens with that encounter. If there are no unlocked encounters for the current date associated with the template, you can creat
