Element	Description
Assessment/Plan Expanded View option	Saved diagnosis details appear in a grid format similar to how it appears on the SOAP template.
Follow Up	Opens the Follow-up/Disposition template to document information surrounding a required appointment or placement. To document follow-up details, see the topic
Postoperative Quality Reporting link	Opens the Postoperative Reporting template to document if the patient has had an unplanned reoperation, hospital readmission, or surgical site infection within 30 days of the operation
Q	Reveals the details required for successful reporting of the Quality Program measure for Unplanned Reoperation within the 30 Day Postoperative Period
Counseling Details link	Opens the template to record tobacco cessation counseling that is concluded in a time frame of three minutes or less
Quick Task	Sends the order to the configured task recipient. Note: If no recipient has been configured on Practice Config practice template, the Quick Task Setup template opens to apply this configuration in runtime. For more information on how to set up task recipients at the practice level, go to NextGen Healthcare Success Community, and download the Practice Configuration Template User Guide for NextGen® Adaptive Content Engine.
	To set up a new quick tasking action, select to open the Quick Task Setup template.

Documenting Assessment Plan Details

You can document your impressions and comments, differential diagnoses, and patient or provider plan details for an assessment using the A/P Details tab. You can update an assessment with the following details: Add or update an assessment in the Today's Assessment grid. Document a health promotion plan for the patient. Review the previous patient and provider plan history details, and update them as required. Review and update exceptions on the patient plans. Document an Assessment Plan

You can document patient plan and provider plan details for the patient's assessment. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. Open the Intake template, select the specialty as Family Practice and the visit type as Office Visit, and then select My Plan. Select the A/P Details tab.The Assessment Plan Details template opens. Select an assessment from the Today's Assessment grid.The existing assessment details appear in the corresponding fields. Enter the details or use My Phrases or Common Phrases for the impression, differential diagnosis, and patient and provider plans in the respective sections. To enter follow-up appointment and disposition information, select Follow Up. To document counseling educational details and factors, select the Counseling Details. Enter Assessment Plan Details

Open NextGen® Enterprise EHR. On the Assessment Plan Details template, select the A/P Details tab. To add or change a diagnosis in the Today’s Assessments grid, select Add or Edit to open the Add or Update Assessment template. To change the current working assessment, in the Today's Assessments grid, select the diagnosis being addressed for this encounter. The selected diagnosis appears in the Selected Dx field. It becomes the new working assessment that carries forward to the other templates that share the assessment. Enter details manually or use My Phrases or Common Phrases for the impression, differential diagnosis, and patient and provider plans in the respective sections. You can use links and buttons on the Referrals Order template to complete the workflow easily.
