You can document a patient's health promotion plan on the Health Promotion Plan template. You can document the following details on the Health Promotion Plan template. BMI plan Depression plan Hypertension plan Pain management plan Fall risk Screening tools and follow-up care plans Functional and Cognitive Assessment Status Health Promotion Plan You can select the information icon to open the Information Display window, which displays all the details required for successful quality reporting. For adult patients, you can document the diet, physical activity, and referral details. For patients younger than 18 years, you must document the diet and physical activity details. The Exceptions link allows you to manage the exceptions on the Exceptions/Measures template. This link appears in all panels except Functional and Cognitive Assessment Status. Patient Care Plans

You can create a care plan to address patients' health concerns and monitor their progress toward resolving specific concerns. On the Care Plan template, you can do the following: Define and maintain a care plan for a patient that addresses health concerns, goals, interventions, and outcomes Record the functional and cognitive status of the patient Define, track, and review interventions by multiple providers and agencies Track the overall progress of the patient toward achieving defined goals The Care Plan template also enables you to meet the 2015 edition certification criterion, which requires a recording, changing, accessing, creating, and receiving care plan information. You can also create a Care Plan document from the Care Plan template and save the document as part of the encounter history. Manage Health Concerns

The first step in managing patient care is to identify and add the patient health concerns that you, as the provider, would like to address using a care plan. Prior to using the template, you must have entered health care concerns for the patient, either during the current encounter or during prior encounters. Allergies, smoking status, alcohol or drug use, family history, vital signs, and so on would appear as health concern categories on the Care Plan template. Care Plan Components

You can create a care plan to address a patient's health concerns and to monitor their progress toward resolving the identified concerns on the Care Plan template. The care plan consists of following components: Field Description Type The type of care plan Status The status of the care plan Intent The intent of the care plan Panel Description Add/Edit Health Concerns Consolidates categorized health concerns from several sources within the application Health concern categories include but are not limited to allergy, vital signs, social history, smoking and tobacco use. Document Functional and Cognitive Assessments

You can document a patient's functional and cognitive assessments in the Functional and Cognitive Assessment Status panel. The data entered in the Functional and Cognitive Assessment Status panel is shared with the Health Promotion Plan, Medicare Preventive HPI, Screening Summary, and Care Plan templates to measure compliance and for surveillance reporting. Open NextGen® Enterprise EHR. Open the Intake template. Expand the Vital Signs panel, and then select the Health Promotion Plan link. The Health Promotion Plan template opens. Expand the Functional and Cognitive Assessment Status panel. Functional and Cognitive Assessment Status For Functional Status and Cognitive Status, do the following: If there is no change in the functional status of the patient, select No Change. If there is a change in the functional status or cognitive status of the patient, select Change. Select the Status field, and select the appropriate option from the list. Patient Care Team Members

You can use the Patient Care Team Members panel to view and manage a list of providers, agencies, organizations, care takers, and other individuals involved in the ongoing care of the patient. You can create and update patient care team details, either from the Patient Care Team Members panel of the Care Plan template or from the Care Team Members template. From the Patient Care Team Members panel, you can: Create and update patient care team. Add, update, and remove members and their details from the patient care team. Import practice care team to patient care team as one of the member.
