Forms

You can create and modify templates to standardize patient forms that are used by your practice. The form templates determine the structure for patient forms, such as where certain information must be placed on the form. For example, if you have pre-printed fee tickets, you can set up a template to your specifications. You can create label templates for mailing labels, chart labels, form labels, or any other labels you need. To design your own forms, you need to access the Forms Templates master file from File Maintenance > System > Forms master file. Print Enhanced Forms

Wherever you can print Forms in NextGen® Enterprise PM, you can select to print Forms Enhanced. The Print Forms window includes separate tabs for each type of form. Note: The below procedure is also used to print Fee Tickets or Patient Data Sheets that are setup as Enhanced Forms in File Maintenance. This procedure replaces the legacy Print > Fee Tickets or Print > Patient Data Sheets menu options that are used to print the "standard" version of these forms. In NextGen® Enterprise PM, select the Print > Forms option from a right-click menu. The Print Forms window appears. Select the Forms Enhanced tab. The Form Group field defaults to (All Forms). This allows you to select and print forms from any group. Select a specific Form Group to select and print forms from that group only. In the Forms section, select the form(s) to be printed. All forms are selected by default. Select the Sort button to modify the sort for a specific form, if needed. The sort is used only when the form is print

Setting Up the Printing Sort Order Criteria for Form and Label Templates

The printing sort order criteria for forms and labels are set up at two levels: The Form and Label Template master files level, which is the default printing sort order for the selected form or label template. Batch mode printing in NextGen® Enterprise PM, which allows you to change the default printing sort order of the data fields that were selected in the Form and Label Template Master Files. For more information, see Change the the Default Sort Order when Printing in Batch Mode. Print Enhanced Forms

You can create enhanced forms in File Maintenance. These include forms for electronic signatures, patient correspondence for documentation purposes, and customized options such as alternate fee tickets and patient data sheets. Wherever forms can be printed in NextGen® Enterprise PM, you can choose to print Enhanced Forms. Note: You can print Fee Tickets or Patient Data Sheets that are setup as Enhanced Forms in File Maintenance. Printing enhanced forms replaces the legacy Print > Fee Tickets or Print > Patient Data Sheets menu options that are used to print the standard version of these forms. Open NextGen® Enterprise PM. Select File > Print Forms > Forms or select the desired patient from Encounter Lookup window, and then right-click, and select Print > Forms. The Print Forms window appears. Set Up a Default Sort Order for Printing Forms and Labels

Forms and Labels can be created in File Maintenance and then printed for an individual patient, or for multiple patients in NextGen® Enterprise PM. You can set up a default sort order for Forms or Label Templates, which will be applied during previewing or batch printing. Open File Maintenance. From the Master Files section, select System , then select PM, and then select either Forms or Label Templates.The Forms List or Label Templates List window appears. From the list, double-click to open the desired Form or Label Template.The Form Design or Label Design window appears. From the design toolbar, select the Template Sorting Preference "" button.The Template Sorting Preference window appears. All data fields included on the form or label are listed in the Available Data Fields section. Select the data field from the Available Data Fields list, and then select the left and right arrow buttons to add or remove data fields to the Selected Data Fields section to be used in the default sor
