Forms and Labels

Forms and Labels can be created in File Maintenance and then printed for an individual patient, or for multiple patients in NextGen® Enterprise PM.

Any type of form that is used by a practice can be created in the Forms master file, or the Forms Enhanced master file. Some forms may be used to gather information from a patient, such as an intake form. Other forms may be used to provide information to a patient or to their insurance provider. Each form is created using a combination of free-text verbiage for static information on the form, and data fields that are automatically populated with patient-specific information when the form is printed.

Labels that are used by a practice can be created in the Label Templates master file. Examples include mailing labels, chart labels and specimen labels. Similar to forms, each label is created using a combination of free-text verbiage for static information on the label, and data fields that are automatically populated with patient-specific information when the label is printed.

  • Form and Label Templates Printing Sort Order You can specify the form fields that you want to use to sort a form or label template based on the form fields used to create the form and label template.
  • Forms Enhanced Forms Enhanced allows you to create more advanced forms than those created in the legacy Forms master file.
  • Printing Labels Labels can be printed and used to address envelopes for mailed patient correspondence, label patient charts, or identify lab specimens.
  • Printing Fee Tickets A Fee Ticket is a form than can be printed and given to the provider to document diagnosis codes and procedure codes for a patient's encounter.
  • Printing Patient Data Sheets A blank Fee Ticket can be printed without patient-specific information.
