Forms Enhanced

Field	Description
Form Group	Lists of new forms created and saved into groups; the groups can be used to categorize forms (for example, Consent Forms) or group them together form printing purposes (for example, New Patient Registration Forms).
Form Name	Displays the name of the form
Form Description	Displays the form description
Show Hidden Forms	When check box is selected, you can see all forms created including hidden forms. Note: By default, hidden forms are not displayed.
Authored By	Displays who created the form as well as the date and time the form was created.
Last Modifier	Displays who last modified or made changes to the form as well as the date and time last modified.
Practices	Forms that were created for a specific practice. Although, forms can be shared with other practices by granting Practice Access.

Create New Enhanced Form

Form Groups enable you to categorize forms for organizational and printing purposes. You must create at least one Form Group before you can save an Enhanced Form. After a form group has been created, you can also edit, copy, hide, delete and unhide form groups. Open File Maintenance. Select Edit Form Group from the Forms Enhanced menu. Select New. Enter a name for the group. Select OK. Note: If All Forms is selected in the Form Group field, you are not able to modify the practice list displayed. Forms Enhanced

You can create forms in the Forms Enhanced window for electronic signatures, patient correspondences for documentation purposes and more customized options for alternate fee tickets and patient data sheets. Forms Enhanced also allows you to create form groups, allowing you to print multiple pages of a form and multiple forms at once. To create an Enhanced Form, you must first create at least one Form Group. Electronic Signature in Forms Enhanced

The Electronic Signature feature in Forms Enhanced enables you to electronically sign multiple forms or at multiple locations within the same form, for an account or a patient. You must have an electronic signature pad properly installed and configured to obtain electronic signatures. You must add a System Esignature data field in the Forms Enhanced master file in File Maintenance to enable this feature in NextGen® Enterprise PM. You can electronically sign documents shared between different practices; however, you cannot electronically sign documents when generating a blank form from the File > Print menu or when selecting forms for multiple patients. Print Enhanced Forms

Wherever you can print Forms in NextGen® Enterprise PM, you can select to print Forms Enhanced. The Print Forms window includes separate tabs for each type of form. Note: The below procedure is also used to print Fee Tickets or Patient Data Sheets that are setup as Enhanced Forms in File Maintenance. This procedure replaces the legacy Print > Fee Tickets or Print > Patient Data Sheets menu options that are used to print the "standard" version of these forms. In NextGen® Enterprise PM, select the Print > Forms option from a right-click menu. The Print Forms window appears. Select the Forms Enhanced tab. The Form Group field defaults to (All Forms). This allows you to select and print forms from any group. Select a specific Form Group to select and print forms from that group only. In the Forms section, select the form(s) to be printed. All forms are selected by default. Select the Sort button to modify the sort for a specific form, if needed. The sort is used only when the form is print

Forms and Labels

Forms and Labels can be created in File Maintenance and then printed for an individual patient, or for multiple patients in NextGen® Enterprise PM. Any type of form that is used by a practice can be created in the Forms master file, or the Forms Enhanced master file. Some forms may be used to gather information from a patient, such as an intake form. Other forms may be used to provide information to a patient or to their insurance provider. Each form is created using a combination of free-text verbiage for static information on the form, and data fields that are automatically populated with patient-specific information when the form is printed. Labels that are used by a practice can be created in the Label Templates master file. Examples include mailing labels, chart labels and specimen labels. Autoflow Sequence - Printing Enhanced Forms

The Autoflow sequence allows Enhanced Forms selection during both check-in and checkout setup. Prerequisite: Enhanced Forms have been created in the Forms Enhanced master file in File Maintenance. To include Enhanced Forms during the Autoflow sequence, do the following: When the Autoflow process is being setup for check-in or checkout, you must enter a green check in the Print column for a specific Form Group. This allows all forms within that group to automatically print as part of the autoflow sequence. You can pick which form(s) to be printed by not selecting a specific Form Group (no green check indicated).
