Printing Patient Data Sheets

The data sheet includes demographic, responsible party (guarantor), primary insurance and secondary insurance information for the patient. You can use the Patient Data Sheet as an intake form to update the patient's demographic and financial information. There are two forms available: Patient Data Sheet and Patient Data Sheet Detail. The detail version of the form includes additional patient information that is not included on the other form. Patient Data Sheets can be printed from the following areas in NextGen® Enterprise PM: File Menu (blank forms only) Appointment Book Appointment Lookup Patient Chart > Encounters Tab Encounter Lookup Print a Patient Data Sheet from the Appointment Book You can print Patient Data Sheet from a patient's appointment in the Appointment Book. Print Patient Data Sheets from Encounter Lookup

Patient Data Sheets can be printed for one or more patients from the Encounter Lookup window. From the main toolbar, select the Encounter button. The Encounter Lookup window appears. Enter Search Criteria as needed to locate the desired patient encounters. Select Find. All encounters that match the search criteria entered display in the Encounter List section at the bottom of the window. From the Encounter List, select all encounters for which a patient data sheet is to be printed. Right-click and select Print, and then one of the following: Patient Data Sheet Patient Data Sheet Detail The Patient Data Sheet preview window appears. To print the form to the default printer, select the Quick Print button on the toolbar. Note: The default printer for Patient Data Sheets is defined in Practice Preferences > Printing. Print a Patient Data Sheet from the Patient Chart

A Patient Data Sheet can be printed from a patient's encounter in the Patient Chart. Access the Patient Chart for the desired patient. Select the Encounters tab. In the Encounters section on the left, select the desired encounter. Right-click and select Print, and then one of the following: Patient Data Sheet Patient Data Sheet Detail The Patient Data Sheet preview window appears. To print the form to the default printer, select the Quick Print button on the toolbar. Note: The default printer for Patient Data Sheets is defined in Practice Preferences > Printing. If the user has specified a different printer in their General User Preferences > Printing, it overrides the printer defined in Practice Preferences. Print a Blank Patient Data Sheet

A blank Patient Data Sheet can be printed without patient-specific information. From the File menu, select Print Forms, then Patient Data Sheet, and then one of the following: Blank Patient Data Sheet Blank Patient Data Sheet Detail The Patient Data Sheet preview window appears. To print the blank form to the default printer, select the Quick Print button on the toolbar. Note: The default printer for Patient Data Sheets is defined in Practice Preferences > Printing.
