Forms

You can create and modify templates to standardize patient forms that are used by your practice. The form templates determine the structure for patient forms, such as where certain information must be placed on the form. For example, if you have pre-printed fee tickets, you can set up a template to your specifications. You can create label templates for mailing labels, chart labels, form labels, or any other labels you need. To design your own forms, you need to access the Forms Templates master file from File Maintenance > System > Forms master file. Forms and Labels

Forms and Labels can be created in File Maintenance and then printed for an individual patient, or for multiple patients in NextGen® Enterprise PM. Any type of form that is used by a practice can be created in the Forms master file, or the Forms Enhanced master file. Some forms may be used to gather information from a patient, such as an intake form. Other forms may be used to provide information to a patient or to their insurance provider. Each form is created using a combination of free-text verbiage for static information on the form, and data fields that are automatically populated with patient-specific information when the form is printed. Labels that are used by a practice can be created in the Label Templates master file. Examples include mailing labels, chart labels and specimen labels. Setting Up the Printing Sort Order Criteria for Form and Label Templates

The printing sort order criteria for forms and labels are set up at two levels: The Form and Label Template master files level, which is the default printing sort order for the selected form or label template. Batch mode printing in NextGen® Enterprise PM, which allows you to change the default printing sort order of the data fields that were selected in the Form and Label Template Master Files. Add a Patient's Picture to a Form or Label

The application enables you to add a patient's picture to a form or label through Form Templates and Label Templates. After you add the picture to the template, you can print the forms and labels with the picture in NextGen® Enterprise PM. Open File Maintenance. Select System Master Files, then PM, and then either Forms or Label template. The Forms List or Label Templates List window opens. Select the template to which you want to add the picture. Select Open Menu , then select Open. If you want to create a new template, see Creating Forms or Creating Label Templates. The Form Design or Label Design window opens. Select Insert Data Field on the form/label toolbar. The NextGen Data Repository window opens. Double-click Patient Information. A list of patient information fields displays beneath it. Select the Patient Picture data field and drag the item from the list and drop it on the template in the location where you want the picture to print. The data field is inserted in the template
