Complete the Missed Appointment Form

You can document the details of a patient's missed appointment and track action items.

  1. Open the Missed Appointment template.
  2. Click the Provider field, select the name of the provider from the providers window, and then click OK.
    The calendar window opens for selecting the date of the appointment.
  3. Select the date from the calendar window, and click OK.
    A picklist opens for selecting the reason for visit.
  4. Select the reason for visit from the list.
  5. Select the reason for the missed appointment.
  6. Select the appropriate option for # of Prior No Shows.
  7. Enter the actions for clinical staff, response from the provider, and actions of the scheduler.
  8. Click Save & Close in the workflow tab.
    The documented details are saved, and the Missed Appointment template closes.
