You can use the Missed Appointment template to track action items and document the details of a patient's missed appointment.
The following table describes the panels on the Missed Appointment template.

| Clinical Staff | To document the actions for clinical staff and create tasks for clinical staff, scheduler, and provider. | 
| Provider Response | To document the provider's response for the missed appointment, rescheduling the appointment, and terminating the patient from that provider’s care or from the practice. | 
| Scheduler Actions | To track contact attempts with the patient, any scheduling notes, and documents sent to the patient. |
