Documenting Visit Time

You can enter the visit time for a patient's visit on the Billable Visit template. On the Billable Visit template, you can use the fields in the Visit Time sections to track the visit time for the evaluation and management (E&M) coding guidelines. Visit time on the Billable Visit template You can do one of the following to enter the visit time in the Start and End fields. To enter the current time, you can click the Clock icon icon. To enter a specific time, you can click the Down arrow icon icon, and enter the time in the time entry window. If you enter time in the Start and End fields, the total visit time is automatically calculated in minutes and displayed in the Total field. You can also manually enter the total visit time in minutes in the Total field if there are no entries in the Start and End fields. Parent topic: Billable Visits

Finalizing a Patient Visit

You can enter a patient's visit details and today's diagnoses, and apply diagnosis codes to procedures to submit charges and finalize the visit. You can use the Finalize Visit template to do the following: Perform dictation Document the visit type and time Add or remove today's diagnoses Document carbon copies information Apply appropriate diagnosis codes to the procedures Submit charges Send the current encounter to the supervising provider Sign off as the supervising provider Generate letters and reports Lock the current encounter Finalize Visit template You can configure the following for the Finalize Visit template. Dictate button and No Dictation check box. You can use the Dictate button to perform dictation. If dictation is not required, you can select the No Dictation check box. Provider Signoff

The provider can sign off on the Billable Visit template if PAQ functionality is disabled. Click the Electronically Signed By check box. Note: If using the PAQ functionality, the signoff field is hidden. Click Finalize Visit button to submit charges from the Finalize Visit template. Parent topic: Billable Visit Sign-off

Sign-Off

Sign Off as Scribe

If the Scribe Encounter checkbox on the Home template is selected for the current encounter, the scribe can sign off before submitting charges or the provider signs off on the Finalize Visit template. Select the Scribe signoff checkbox. Scribe signoff check box on the Billable Visit template This enters the user’s name with a date and timestamp, and generates the document if PAQ functionality is enabled. Note: You can make the scribe's approval required or optional for submitting charges on scribe encounters by using the Finalize Visit Settings panel of the IORTS Practice Configuration template. Billable Visits

The Billable Visit workflow starts on the Billable Visit template. This template is used to document the History of Present Illness (HPI), Physical Exams, and Assessment and Plan portions of an E&M coded visit. It pulls in information from the Intake template. It has three panels: History, Physical Exam, and Assessment and Plan. Documenting Visit Time You can enter the visit time for a patient's visit on the Billable Visit template. Patient History You can review, add, and update the patient histories. Physical Exam Panel Assessment and Plan Panel (Diagnosis/MDM) Billable Visit Sign-off Documenting Billable Visit Counseling You can document the additional counseling time and its associated details for a patient. Finalizing a Patient Visit You can enter a patient's visit details and today's diagnoses, and apply diagnosis codes to procedures to submit charges and finalize the visit.
