View E&M Summary and Coding Guidelines

You can view the details of a patient's visit, medical decision making (MDM), and evaluation and management (E&M) coding guidelines. The fields on the IORTS EM History 2021 and E&M History templates are read-only, and you can view the details from the Billable Visit template on these templates. On the Billable Visit template, select the appropriate visit type in the History panel. Click the E&M Coding link. The E&M history template opens as follows: If the 2023 E&M coding guidelines are applicable, the IORTS EM History 2021 template opens for the NP, Est, and Consult options. Finalize Template

The Finalize template allows the provider to calculate all of the elements that were documented in the Intake, Histories, and SOAP tabs, select the Evaluation and Management (E&M) code level for the visit, and submit the E&M coding to the Superbill module. Patient's General Information on Finalize You can view the general details related to finalizing the workflow of the patient in the General panel. Patient's Assessments on Finalize You can view, add, or edit patient assessments in the Today's Assessment panel. Provider Sign Off You can submit and review the plan of care based on your role in the Provider Sign Off panel. Evaluation and Management Level Summary You can view the summary of a patient's history, exam levels, and coding points for the Evaluation and Management (E&M) code calculation under the 95 or 97 Coding Guidelines in the E&M Level Summary panel. Submit E&M Codes under 95 or 97 Coding Guidelines

You can submit a calculated or selected E&M code to the Superbill under the 95 or 97 Coding Guidelines. Before you begin You must select an assessment in the Today’s Assessments panel to submit an E&M code. Open NextGen® Enterprise EHR. For a patient record, open a new encounter. Open the Finalize template. Expand the Evaluation and Management Coding panel. Evaluation and Management Coding To use a calculated E&M code, you can select the MDM level, or enter the total visit time. Enter any additional visit codes. To meet visit code criteria for Quality Reporting, select a SNOMED® CT code. To override the calculated code, select an E&M code in the code options section at the end of the Evaluation and Management Coding panel. Note: The 99201 code cannot be selected as the code has expired on January 1, 2021. You can submit a calculated or selected Evaluation and Management (E&M) code to the Superbill using the 2021 Coding Guidelines. Before you begin The date of the encounter must be on or after the effective start date entered for the 2021 Coding Guidelines on the Practice Configuration template. The visit type must be one of the visit types impacted by the 2021 guidelines. For more information on the 2021 Coding Guidelines, go to NextGen Healthcare Success Community, and download the E&M Coding User Guide for NextGen® Adaptive Content Engine. Also, you must select an assessment in the Today’s Assessment panel to be able to submit an Evaluation and Management (E&M) code. Billable Visits

The Billable Visit workflow starts on the Billable Visit template. This template is used to document the History of Present Illness (HPI), Physical Exams, and Assessment and Plan portions of an E&M coded visit. It pulls in information from the Intake template. It has three panels: History, Physical Exam, and Assessment and Plan. Documenting Visit Time You can enter the visit time for a patient's visit on the Billable Visit template. Patient History You can review, add, and update the patient histories. Physical Exam Panel Assessment and Plan Panel (Diagnosis/MDM) Billable Visit Sign-off Documenting Billable Visit Counseling You can document the additional counseling time and its associated details for a patient. Finalizing a Patient Visit You can enter a patient's visit details and today's diagnoses, and apply diagnosis codes to procedures to submit charges and finalize the visit. Calculate an E&M Code under 2021 Coding Guidelines on the FTS Finalize Template

You can base the Evaluation and Management (E&M) code calculation on the selected Medical Decision Making (MDM) level or on the total visit time. Open the FTS Finalize template and expand the Evaluation and Management Coding panel. In the Medical Decision Making section, do one of the following: Review prepopulated or manually select the level of complexity associated with the encounter. Enter the total visit time. To calculate the E&M code based on the MDM level, review prepopulated or select the level of complexity associated with the encounter in the Medical Decision Making section. The Medical Decision Making section shows the MDM level based on the calculated levels in the three tables in the Medical Decision - Making Elements panel. To change the MDM level, change the automatically-calculated MDM levels in the MDM tables, or by making corrections in the encounter chart note.
