Documenting Billable Visit Counseling

You can document the additional counseling time and its associated details for a patient. You can use the BV Counseling template to document additional counseling time spent with a patient. The Visit Type is automatically selected based on the data entered on the Billable Visit template. You can also document the details of the counseling given to the patient. Selecting the Counseling Override check box enables you to override the calculated evaluation and management (E&M) code for counseling and enter the total encounter time to determine the E&M level. BV Counseling template If the 2023 E&M coding guidelines are applicable, the Counseling Override check box does not appear on the BV Counseling template, and the Total Encounter Time field is made available to enter the values. Patient History

You can review, add, and update the patient histories. The History panel on the Billable Visit template enables you to select the visit type and review the diagnoses, histories, medications, allergies, and review of systems of a patient. You can also add or update the details of the history of present illness (HPI). The following grids are automatically filled with the data documented on the Intake template. You can add, edit, and remove the data as needed. Billable Visits

The Billable Visit workflow starts on the Billable Visit template. This template is used to document the History of Present Illness (HPI), Physical Exams, and Assessment and Plan portions of an E&M coded visit. It pulls in information from the Intake template. It has three panels: History, Physical Exam, and Assessment and Plan. Documenting Visit Time You can enter the visit time for a patient's visit on the Billable Visit template. Patient History You can review, add, and update the patient histories. Physical Exam Panel Assessment and Plan Panel (Diagnosis/MDM) Billable Visit Sign-off Documenting Billable Visit Counseling You can document the additional counseling time and its associated details for a patient. Finalizing a Patient Visit You can enter a patient's visit details and today's diagnoses, and apply diagnosis codes to procedures to submit charges and finalize the visit. Total Visit Time Calculation

Use this section to configure how visit time is recorded for your practice. You can set it up to use buttons that enter start and end timestamps (12-hours or 24-hours), or so that users enter the total visit time manually (Manual Time). If you select 12H or 24H Start - End, a Start Time button and field appear at the top of the Single Visit template, and Start Time and End Time buttons and fields appear at the bottom of the template. Document Counseling in the Billable Visit

You can document total time spent with patient either by selecting the start and end time, or by entering the total time. On the BV Counseling template, do the following to record the counseling time: Enter the Start time. You can click the clock icon to record the current time or enter the time manually. Enter the End time by clicking the clock or enter the time. The Time Requirements for E&M coding are listed next to the time fields. Alternatively, you can enter the total encounter time by clicking in the Total Encounter Time field and selecting the total time from a picklist. The application calculates the total time automatically. Enter Verbal Counseling. Enter Coordination of Care. Enter the Printed Materials given to the patient. Click the E&M Key button at the bottom of the template. Visit Time

You can document the visit time for patients using one of the following methods based on your practice configuration done in the Single Visit section on the Practice Config practice template. If the application is configured to use the start and end timestamps to calculate total time for the patient visit, a Start Time button and field appear at the top of the Single Visit template, and Start Time and End time buttons and fields appear at the bottom of the template. Start Time and End Time fields on the Single Visit template If the application is configured to use manually entered total time, the Total Active Visit Time field appears at the bottom of the template. Note: If you change the minutes in Total Active Visit Time on the Single Visit template, you must reopen Fee Sheet to ensure correct calculations and documentation for the encounter.
