Lensometry/Refraction Exams

The Lensometry/Refraction panel or section displays current encounter exam findings, as well as findings for the last five encounters, in separate grids for OD and OS on the Prelim Exam - OPH and Provider - OPH tab templates. lensometry refraction panel data The Lensometry/Refraction panel is found on multiple templates, including: Home Page - OPH– displays historical data Prelim Exam – OPH – used for technician workflow Provider – OPH – used for provider workflow Oph EyeTech – used for documenting preliminary exam findings Lensometry and refraction findings are documented on the Oph EyeTech template on the Lensometry/Refraction panel. Print Glasses Prescription for Low Vision

The Glasses Prescription to Print panel on the LV Exam - OPT template displays sets of findings from the different lensometry and refraction exams. You can select or update the values to create the new glasses prescriptions for the patient. After you finalize the prescription information, you can print each prescription separately and give them to the patient. To use a set of findings from the lensometry and refraction exams for the new prescription, select the check box to the left of the set of findings values you want to use. The exam values in the set are copied to the New Rx 1 prescription fields. Make changes to the prescription, as needed, and enter Comments for New Rx 1. Click Print Rx 1 to generate a prescription for the patient. To copy the values from New Rx 1 to New Rx 2, select Copy Rx1 to Rx 2. Make changes to the prescription, as needed, and enter Comments for New Rx 2. Click Print Rx 2 to generate a prescription for the patient. Parent topic: Lensometry/Refraction Panel

Lensometry and Refraction Exam Findings Documented

The Lensometry/Refraction panel has several sections for documenting glasses prescriptions and refraction. Lensometry/Refraction Section Description Lensometry Eyeglass prescriptions are documented. Autorefraction Results of refraction measured using an Auto-refractor are documented. Manifest RX Results of refraction measured while eye is able to accommodate (in its natural state, not dilated) are documented. Cycloplegic Results of refraction measured after the administration of cycloplegic eye drops to temporarily paralyze the focusing muscles of the eye are documented. Information Documented for an Eyeglasses Rx

The Eyeglasses Rx - OPH template includes several sections that can be used to write a prescription for glasses, including the following: Panel/Section Description Lensometry Use this section to document an exam or review and update findings from an exam performed previously during the encounter, such as on the Oph Eyetech template. Up to three prescriptions can be documented; select the check boxes to the left of a set of data fields for each eye to copy the values to the corresponding fields for New Rx 1 in the Glasses Prescription to Print section. Auto-refraction Use this section is used to document objective and subjective measurements taken by an auto-refractor; select the check boxes to the left of a set of fields for each eye to copy the set of objective and/or subjective measurements to the corresponding New Rx 1 fields. Manifest and Cycloplegic Refraction Use this section to document up to two sets of manifest refraction measurements and one set of cycloplegic refraction meas

Low Vision Exam

The provider can document the exam for a low vision visit on the LV Exam - OPT template. You can review the testing performed by the technician and perform additional exams as needed. Enter Visual Acuity Findings Document an External Exam You can document the external exam of an eye in the External Exam panel. Document a Slit Lamp Exam Document a Posterior Exam Enter Cover Test Findings Enter Nystagmus Findings You can record findings associated with nystagmus in the Nystagmus panel. Record Color Vision Findings You can record color vision deficiencies in the Color Vision panel. Document Corneal Light Reflex Test Results You can enter your findings from the Corneal Light Reflex test in the Corneal Light Reflex Test panel. Record the Amsler Grid Test Results Document Contrast Sensitivity Test Findings Document Low Vision Objective Notes Optometry providers can document objective notes in the LV Objective Notes panel. Review Historical Lensometry and Refraction Findings

You can view historical exam findings for lensometry and refraction exams. Click the Historical Lensometry/Refraction link in the Lensometry/Refraction exam panel. The OPH - Lensometry/Refraction History template opens and displays lensometry and refraction data for the current patient. oph lensometry refraction history template Select the option corresponding to the desired refraction exam to view, Manifest Rx or Cycloplegic Rx.The Refraction History grid refreshes and displays historical findings for the selected refraction exam.
