link in the
Lensometry/Refraction
exam panel.
The
OPH - Lensometry/Refraction History
template opens and displays lensometry and refraction data for the current patient.
Select the option corresponding to the desired refraction exam to view,
Manifest Rx
or
Cycloplegic Rx
.
The
Refraction History
grid refreshes and displays historical findings for the selected refraction exam.
Click
Close
to exit the template.

Document a Returning Patient’s Current Glasses Prescription
Before you begin To perform these steps, the patient must be a returning patient with prescription information for eyeglasses from a previous encounter. For a returning patient with historical data, you can copy a patient’s previous eyeglasses prescription by selecting a set of Lensometry, Refraction, or glasses prescription values from the OPH – Lensometry/Refraction History template to the current encounter. Click the Historical Lensometry/Refraction link. The OPH – Lensometry/Refraction History template opens. oph lensometry refraction hsitory For the Refraction History grid, select the option to display either Manifest Rx or Cycloplegic Rx. Select the desired row containing the values to copy from one of the grids: Lensometry History, Refraction History, or Glasses Prescription History. Lensometry and Refraction Exam Findings Documented
The Lensometry/Refraction panel has several sections for documenting glasses prescriptions and refraction. Lensometry/Refraction Section Description Lensometry Eyeglass prescriptions are documented. Autorefraction Results of refraction measured using an Auto-refractor are documented. Manifest RX Results of refraction measured while eye is able to accommodate (in its natural state, not dilated) are documented. Cycloplegic Results of refraction measured after the administration of cycloplegic eye drops to temporarily paralyze the focusing muscles of the eye are documented. Document a Lensometry Exam
Expand the Lensometry/Refraction panel if not already expanded. Select the Glasses Rx Verified By check box to indicate that you verified the glasses prescription. The current user name fills the corresponding field. In the Lensometry section, enter OD values in the following fields: SPH CYL Axis - enter a numeric value ADD Prism 1 Base 1 Prism 2 Base 2 Repeat step three to enter OS values. Enter Manifest Refraction Findings
The Manifest RX section includes data fields for documenting manifest refraction and manifest visual acuity findings. In the Manifest RX section, enter the values for OD. The Submit charge for refraction check box is selected automatically when refraction values are documented. Enter the manifest visual acuity in the following fields: Dva Dva modifier Nva Nva modifier Repeat steps 1-2 to enter OS values. Enter OU values. Important: If your practice has added multiple SIM codes to the Refraction Billing (RxBL) Service Category in File Maintenance, you have to clear and re-select the Submit charge for refraction billing check box to open a DB picklist listing the configured codes. You can then select the appropriate code. If you do not want to submit charges, clear the Submit charge for refraction check box. Enter Comments. Lensometry/Refraction Exams
The Lensometry/Refraction panel or section displays current encounter exam findings, as well as findings for the last five encounters, in separate grids for OD and OS on the Prelim Exam - OPH and Provider - OPH tab templates. lensometry refraction panel data The Lensometry/Refraction panel is found on multiple templates, including: Home Page - OPH– displays historical data Prelim Exam – OPH – used for technician workflow Provider – OPH – used for provider workflow Oph EyeTech – used for documenting preliminary exam findings Lensometry and refraction findings are documented on the Oph EyeTech template on the Lensometry/Refraction panel.
