Enter Manifest Refraction Findings
The Manifest RX section includes data fields for documenting manifest refraction and manifest visual acuity findings. In the Manifest RX section, enter the values for OD. The Submit charge for refraction check box is selected automatically when refraction values are documented. Enter the manifest visual acuity in the following fields: Dva Dva modifier Nva Nva modifier Repeat steps 1-2 to enter OS values. Enter OU values. Important: If your practice has added multiple SIM codes to the Refraction Billing (RxBL) Service Category in File Maintenance, you have to clear and re-select the Submit charge for refraction billing check box to open a DB picklist listing the configured codes. You can then select the appropriate code. If you do not want to submit charges, clear the Submit charge for refraction check box. Enter Comments. In the Binocular Balance section, enter OD findings for the following: SPH CYL Axis ADD Prism 1 Base 1 Prism 2 Base 2 Dva and mod Nva and mod To copy the OD row to the Glasses Prescription to Print section, select Set Binocular Balance OD For the OS row, enter findings for the following: SPH CYL Axis ADD Prism 1 Base 1 Prism 2 Base 2 Dva and mod Nva and mod To copy the row to the Glasses Prescription to Print section, select Set Binocular Balance OS. Document a Returning Patient’s Current Glasses Prescription
Before you begin To perform these steps, the patient must be a returning patient with prescription information for eyeglasses from a previous encounter. For a returning patient with historical data, you can copy a patient’s previous eyeglasses prescription by selecting a set of Lensometry, Refraction, or glasses prescription values from the OPH – Lensometry/Refraction History template to the current encounter. Click the Historical Lensometry/Refraction link. The OPH – Lensometry/Refraction History template opens. oph lensometry refraction hsitory For the Refraction History grid, select the option to display either Manifest Rx or Cycloplegic Rx. Select the desired row containing the values to copy from one of the grids: Lensometry History, Refraction History, or Glasses Prescription History. Verify the Patient’s Glasses Prescription
Before you begin A prescription for eyeglasses should be entered for a patient before you verify the prescription. After copying the values for a patient’s current eyeglasses prescription, you can verify the prescription and document that you have done so using the Glasses Rx Verified By: check box. The Glasses Rx Verified By: check box appears on the following templates: Prescreening – OPH, Prelim Exam – OPH, Oph EyeTech, Provider – OPH, Eyeglasses Rx – OPH, Eye Motility Detail Peds - OPH, Eye Summary, and Lasik Evaluation – OPH. To document that you verified the glasses prescription, select the Glasses Rx Verified By: check box. The name of the current user appears in the field next to the Glasses Rx Verified By: check box.
