Information Documented for an Eyeglasses Rx
The Eyeglasses Rx - OPH template includes several sections that can be used to write a prescription for glasses, including the following:

Lensometry | Use this section to document an exam or review and update findings from an exam performed previously during the encounter, such as on the Oph Eyetech template. Up to three prescriptions can be documented; select the check boxes to the left of a set of data fields for each eye to copy the values to the corresponding fields for New Rx 1 in the Glasses Prescription to Print section. |
Auto-refraction | Use this section is used to document objective and subjective measurements taken by an auto-refractor; select the check boxes to the left of a set of fields for each eye to copy the set of objective and/or subjective measurements to the corresponding New Rx 1 fields. |
Manifest and Cycloplegic Refraction | Use this section to document up to two sets of manifest refraction measurements and one set of cycloplegic refraction measurements; values in the first set of fields can be copied to the second set of fields and changes made; select the check boxes to the left of a set of fields for each eye to copy the set of manifest refraction measurements to the corresponding New Rx 1 fields. |
Glasses Prescription from Interface Instrument | Fields in this section display measurements collected from an instrument that is interfaced with the system; select the check boxes to the left of a set of fields for each eye to copy the set of values for standard glasses to the corresponding New Rx 1 fields. |
Glasses Prescription to Print | This section has sets of fields for up to three new prescriptions that display values copied from sets of lensometry and refraction findings in the sections above and modified if appropriate; providers can review a prescription and copy it from the New Rx 1 fields to the next set of New Rx fields, and make changes as needed for a second or third prescription. After finalizing the prescription(s) and selecting details for the glasses and lenses to be ordered for the patient, each prescription can be printed out separately and given to the patient. |
Near Vision Glasses | Used specifically for near vision glasses, allowing providers to give a fourth prescription to the patient. |
