Panel/Section	Description
Lensometry	Use this section to document an exam or review and update findings from an exam performed previously during the encounter, such as on the Oph Eyetech template. Up to three prescriptions can be documented; select the check boxes to the left of a set of data fields for each eye to copy the values to the corresponding fields for New Rx 1 in the Glasses Prescription to Print section.
Auto-refraction	Use this section is used to document objective and subjective measurements taken by an auto-refractor; select the check boxes to the left of a set of fields for each eye to copy the set of objective and/or subjective measurements to the corresponding New Rx 1 fields.
Manifest and Cycloplegic Refraction	Use this section to document up to two sets of manifest refraction measurements and one set of cycloplegic refraction measurements; values in the first set of fields can be copied to the second set of fields and changes made; select the check boxes to the left of a set of fields for each eye to copy the set of manifest refraction measurements to the corresponding New Rx 1 fields.
Glasses Prescription from Interface Instrument	Fields in this section display measurements collected from an instrument that is interfaced with the system; select the check boxes to the left of a set of fields for each eye to copy the set of values for standard glasses to the corresponding New Rx 1 fields.
Glasses Prescription to Print	This section has sets of fields for up to three new prescriptions that display values copied from sets of lensometry and refraction findings in the sections above and modified if appropriate; providers can review a prescription and copy it from the New Rx 1 fields to the next set of New Rx fields, and make changes as needed for a second or third prescription. After finalizing the prescription(s) and selecting details for the glasses and lenses to be ordered for the patient, each prescription can be printed out separately and given to the patient.
Near Vision Glasses	Used specifically for near vision glasses, allowing providers to give a fourth prescription to the patient.
Other Readings	Used for documenting Auto-keratometry measurements. Pachymetry findings appear in the grid.

Document Prescriptions for Glasses

Depending on the practice, workflow on the Eyeglasses Rx - OPH template may begin after intake or after preliminary exams performed by a technician. Any information previously documented during the encounter appears in the corresponding data fields on the Eyeglasses Rx - OPH template. This information can be reviewed and updated as appropriate. To view historical lensometry, refraction, and glasses prescription details for the patient, in the Manifest and Cycloplegic Refraction section, click the Historical Lensometry/Refraction link. Technicians can perform lensometry testing and document findings on the Prelim Exam - OPH template or on the Eyeglasses Rx template. On the Eyeglasses Rx – OPH template, the first set of fields are prescription fields for Glasses 1. Providers can review and update technician exam findings or document a new exam using the fields for Glasses 2 or Glasses 3. For returning patients with historical data, you can copy previously documented values and update as

Eyeglasses Rx

The Eyeglasses Rx – OPH template is used for documenting and refining prescriptions for eyeglasses. Lensometry/Refraction Exams

The Lensometry/Refraction panel or section displays current encounter exam findings, as well as findings for the last five encounters, in separate grids for OD and OS on the Prelim Exam - OPH and Provider - OPH tab templates. lensometry refraction panel data The Lensometry/Refraction panel is found on multiple templates, including: Home Page - OPH– displays historical data Prelim Exam – OPH – used for technician workflow Provider – OPH – used for provider workflow Oph EyeTech – used for documenting preliminary exam findings Lensometry and refraction findings are documented on the Oph EyeTech template on the Lensometry/Refraction panel. Document a Returning Patient’s Current Glasses Prescription

Before you begin To perform these steps, the patient must be a returning patient with prescription information for eyeglasses from a previous encounter. For a returning patient with historical data, you can copy a patient’s previous eyeglasses prescription by selecting a set of Lensometry, Refraction, or glasses prescription values from the OPH – Lensometry/Refraction History template to the current encounter. Click the Historical Lensometry/Refraction link. Select the desired row containing the values to copy from one of the grids: Lensometry History, Refraction History, or Glasses Prescription History. Print Glasses Prescription for Low Vision

The Glasses Prescription to Print panel on the LV Exam - OPT template displays sets of findings from the different lensometry and refraction exams. You can select or update the values to create the new glasses prescriptions for the patient. After you finalize the prescription information, you can print each prescription separately and give them to the patient. To use a set of findings from the lensometry and refraction exams for the new prescription, select the check box to the left of the set of findings values you want to use. The exam values in the set are copied to the New Rx 1 prescription fields. Make changes to the prescription, as needed, and enter Comments for New Rx 1. Click Print Rx 1 to generate a prescription for the patient. To copy the values from New Rx 1 to New Rx 2, select Copy Rx1 to Rx 2. The Lensometry/Refraction panel enables you to enter and review lensometry, refractions, and print glasses prescriptions. You can also use the Historical Lensometry/Refraction link to view historical exam findings. Document a Lensometry Exam Document Auto-Refraction Values Enter Manifest Refraction Findings Record Cycloplegic Refraction Findings You can record the Cycloplegic refraction findings in the Cycloplegic Rx section. Record Binocular Balance Enter Trial Frame Refraction Findings You can enter the trial frame refraction values and copy them to the Glasses Prescription to Print section in the Trial Frame section of the Lensometry/Refraction panel.
