Document Prescriptions for Glasses

Depending on the practice, workflow on the Eyeglasses Rx - OPH template may begin after intake or after preliminary exams performed by a technician. Any information previously documented during the encounter appears in the corresponding data fields on the Eyeglasses Rx - OPH template. This information can be reviewed and updated as appropriate.
  1. To view historical lensometry, refraction, and glasses prescription details for the patient, in the Manifest and Cycloplegic Refraction section, click the Historical Lensometry/Refraction link.
  2. Technicians can perform lensometry testing and document findings on the Prelim Exam - OPH template or on the Eyeglasses Rx template.
    On the Eyeglasses Rx – OPH template, the first set of fields are prescription fields for Glasses 1.
  3. Providers can review and update technician exam findings or document a new exam using the fields for Glasses 2 or Glasses 3.
    • For returning patients with historical data, you can copy previously documented values and update as appropriate.
    • To manually document an exam, the first set of fields for Lensometry on Eyeglasses Rx - OPH are prescription fields for Glasses 1:
      • Desc – select a description for the prescription
      • Status – select the status for wearing the prescription as described by the patient and the eyes affected
      • Type – select the type of glasses for the prescription
      • OD parameters as appropriate: SPH, CYL, Axis, ADD, Prism 1, Base 1, Prism 2, Base 2, PD, and Binocular PD
      • OS parameters as appropriate: SPH, CYL, Axis, ADD, Prism 1, Base 1, Prism 2, Base 2, PD, and Binocular PD

  4. Perform Auto-Refraction testing and document results or review/update previously documented results. There are fields for objective and for subjective refraction findings from an auto-refractor.
    • Technicians can document objective auto-refractor findings and subjective on the Prelim Exam - OPH template or on the Eyeglasses Rx template.
    • Providers can review and update technician exam findings.
    • The first set of fields are prescription fields for Objective auto-refraction and the second set are for Subjective auto-refraction findings.
  5. Perform Manifest refraction testing and document results or review/update previously documented results; there are fields for up to two sets of manifest refraction data fields.
    • Technicians can document Manifest refraction findings on the Prelim Exam - OPH template or on the Eyeglasses Rx template.
    • Providers can review and update technician exam findings or document a new exam using fields for Manifest refraction 2 by first selecting the Copy MR1 to MR2 check box.
    • The first set of fields are prescription fields for Manifest refraction 1.

  6. If appropriate, perform Cycloplegic refraction testing and document results or review/update previously documented results.
    • Technicians can document Cycloplegic refraction findings on the Prelim Exam - OPH template or on the Eyeglasses Rx - OPH template.
    • Providers can review and update technician exam findings or document a new exam using fields for Cycloplegic refraction.
  7. If set up to do so, perform testing for standard glasses using an interfaced instrument, and bring results into NextGen® Enterprise EHR into the Prescreening – OPH template and the fields in the Glasses Prescription from Interface Instrument section. Technicians can review and update the imported standard glasses prescription findings in the Glasses Prescription from Interface Instrument section.
  8. Glasses Prescription to Print – this is where sets of findings from the different lensometry and refraction exams are selected and reviewed/updated to create the new glasses prescriptions for the patient.

    Finalizing the prescription starts with the New Rx 1 section.

    1. To use a set of findings from the lensometry and refraction exams for the new prescription, select the check box to the left of the set of findings values you wish to use. You to use these values as a starting point for the new prescription.
      The exam values in the set are copied to the New Rx 1 prescription fields near the bottom of the template.
    2. Changes can be made to update the values that appear in the New Rx 1 data fields. Enter details for the glasses:
      • Type – select the type of glasses the prescription is for
      • Material – select the type of material the lenses should be made of
      • Treatment – select any lens treatments to add
      • Expiration – select the appropriate option for when the prescription expires: one year or two years. The date field automatically fills with a corresponding date based on the current encounter date.
    3. Select order instructions for the lenses, and a statement builds in the large Comments text field:
      • Match base curves
      • Match current lens materials
      • Match optical centers
      • Match PD’s
      • Match current prism
      • Match center thickness
    4. If this prescription is for post-operative glasses:
      • Select the Post-Operative RX check box and then select the check box for the surgical eye.

        The corresponding Diagnosis field displays the diagnosis associated with the surgery. If a diagnosis does not appear, click the field and select using the diagnosis search window.

      • Click the corresponding Surgery Date field and select the date surgery was performed from the onscreen Calendar.
      • Click the corresponding Visual Acuity field and select visual acuity from the list.
    5. To print out a copy of the prescription, click Print RX 1.
    6. To use the New Rx 2 prescription fields, select the Copy Rx 1 to Rx 2 check box to copy the values that appear in the New Rx 1 fields to the New Rx 2 fields.
      • Follow steps outlined for New Rx 1.
      • To print out a copy of the prescription, click Print RX 2.
    7. To use the New Rx 3 prescription fields, select the Copy Rx 1 to Rx 3 check box to copy the values that appear in the New Rx 1 fields to the New Rx 3 fields.
      • Follow steps outlined for New Rx 1.
      • To print out a copy of the prescription, click Print RX 3.

  9. The Near vision glasses section is used for prescriptions for near vision glasses; there are fields for one set of near vision manifest refraction findings.

    Perform testing for near vision and document results or review and update previously documented results. Testing can be performed for near vision glasses using an interfaced instrument. Bring the results into NextGen® Enterprise EHR, and then review and update the previously documented results.

    Providers can review and update technician exam findings or document a new exam using fields for near vision refraction.

    1. For each eye, select the check boxes to the left of the findings to use for the prescription.
    2. Make changes as needed to update the values that appear in the data fields in the New near vision glasses prescription to print section.
    3. Enter details for the glasses and select order instructions for the lenses.
    4. If this is a prescription for postoperative glasses, select the Post-Operative RX check box and document surgery-related details.
    5. To print out a copy of the prescription, click Print RX 4.

  10. The Other Readings section has fields for documenting Auto-keratometry measurements and a grid displaying Pachymetry findings. To document Auto-keratometry for each eye, document the following as appropriate:
    • R1 and R1 Axis, R2 and R2 Axis - Click the first reading field, such as R1, and enter using the on-screen numeric keypad or keyboard and click OK; if no value is needed, click OK without selecting a value. An onscreen numeric keypad automatically opens for the corresponding Axis field.
    • Avg – The average value for the R1 and R2 values calculates automatically.
    • Cornea size – Click the Cornea size field and select a value from the list. If a value is not needed, click the top blank line.
    • Pupil size – Click the Pupil size field and select a value from the list. If a value is not needed, click the top blank line.

    Pachymetry findings are documented on the Oph EyeTech template and appear in the Pachymetry grid. Click the Other Tests link in the left navigation pane to open the Oph EyeTech template.
