Document Provider Communication
Provider communications can be documented on the Provider Communication panel or template.
- Do one of the following to open the Provider Communication panel or template:
- On the Assessment/Plan - OPH template, expand the Provider Communication panel.
- On the Provider - OPH template, scroll to the bottom and click the Provider Comm. button.
The Provider Communication template opens. - The Date field automatically displays the current date. To select another date, click the Date field and select a date from the on-screen calendar.
- Click each of the following fields as applicable and select a value from the list. To enter a value not listed, select the blank line at the top and type the value.

- Communication regarding
- Type the name of the individual communicated.
- To document a physician, click the Name field and select from the list of physicians in the system or select the top blank line and type the name.
- The Diagnosis and Code display a previously documented assessment automatically.
- If the communication attempt was completed
- Select the Communicated check box.
- Click the via field and select the method from the list or select the top blank line and type the method.
- If the communication attempt was not successful or not completed
- Select the Not communicated check box.
- Enter a reason by clicking the Reason field and selecting from the list or select the top blank line and type the reason.
- Enter any comments in the Comments field.
- Click the Add/Update button.
The documented communication appears in the Provider Communication grid and includes the following information for the audit log: the Date and Time the record was created, and the Edited by field displays the name of the user who created the record.
