You can document the communication details by using the Provider Communication template.
- Open NextGen® Enterprise EHR.
- On the Intake or SOAP template, select Provider Comm
in the Navigation Buttons section.
The Provider Communication template opens, and the Date field displays today’s date. You can change the date if necessary.
- Select each of the following fields as appropriate, and select from a list. To enter a value manually, select the upper blank line and enter the value.
Note: When you select high osteoporosis fracture risk from the
Communication regarding list, it should open the diagnosis search window. If not, you can correct this on the
Custom Dbpicklists system template by changing the value of
Qualifier 1 to
Y for yes for high osteoporosis fracture risk. For more information on configuring custom DB picklists, go to

- To document a physician, select Name, and then select from the list of physicians or select the first blank line to enter the name.
- The Diagnosis and Code display a previously documented assessment automatically.
- If the communication attempt is completed, select the Communicated checkbox, select via, and then select or enter the method.
- If the communication attempt is not successful or not completed, select the Not communicated checkbox, select Reason, and then select or enter a reason.
- Enter your comments.
- Select Add/Update.
The documented communication appears in the Provider Communications grid. In the audit log, Date and Time appear when the record is created, and Edited by displays the name of the user who created the record.
- Select Save & Close.
