Ophthalmology Provider Workflow
Provider workflow may begin at the Home Page - OPH template, where you can review patient information. The provider workflow for documenting a patient exam continues on the Provider - OPH template. Providers can review tests performed by the technician, document the physical exam, and record tests performed during the provider's portion of the encounter. Patient History
You can document the patient's past ocular history, blood sugar control, and other ophthalmic history on the Histories - OPH template. For more information about documenting a patient's history, go to NextGen Healthcare Success Community and download the latest Getting Started User Guide for NextGen® Adaptive Content Engine. Patient history types include the following: Past ocular history – includes ocular procedures for the patient Medical/Surgical/ Interim Family history- significant family history Social history- includes tobacco, alcohol, and caffeine use; general social history information, lifestyle, and occupational information Confidential History link- located in the Social History panel; Includes alcohol and drug information, psychiatric care information, abuse and /or incarceration, sexual practices, and sexually transmitted infections Glucose Monitoring – used to follow a patient’s history of blood sugar control and includes HbA1C and Glucose testing values by the patient o

Begin a New Ophthalmic Encounter
When a patient arrives, intake staff can open the patient chart and the Home Page - OPH template to begin documenting the new encounter for Ophthalmology and Optometry. You can review historical patient data and update allergies and medications. Open the Home Page - OPHtemplate. The template opens with the specialty and visit type defaulted to Ophthalmology and Office Visit. To change the specialty, click the Specialty field, and select a value from the list. To change the visit type, click the Visit Type field, and select a value from the list. Review the information in the following panels: General- review or update the general patient information Health Monitor- review or update the dates of the most recent testing and procedures Historic HPI Impression/Plan Summary- view the history of the patient's impression or plan information Visual Acuity- view the pateint's visual acuity measurements from previous encounters Orders- view and manage orders IOP- view the pateint's intraocular p

Contagion Risk
The Contagion Risk link is available on every main Ophthalmic template. The Contagion Risk link and the corresponding badge count are used to indicate when the patient is at risk of being exposed to contagions. The Contagion Risk link opens the Contagion Exposure template. The Contagion Exposure template is used to document patient exposure to contagions and any travel to areas with a high risk of exposure to contagions. You can view historical information for risk of exposures to contagions and an exposure log for the patient in the corresponding grids. You can add, update, remove, or deactivate contagion exposure information from the Contagion Exposure History grid when working on an unlocked encounter. When you enter a contagion on the Contagions Exposure template, the contagion is automatically flagged and remains in the badge count until it is deactivated. Complete the Ophthalmology Provider Workflow
For most office visit encounters in general Ophthalmology, the following is a basic outline of the workflow for the provider portion of the visit. To review historical information of the patient, go to the Home Page – OPH template. Go to the Provider – OPH template to document the provider's portion of the encounter. Note: For certain encounters, there are specific single-page visit templates, such as Postoperative Exam – OPH. Select the Provider Summary link to review patient information for today's encounter. Select the Eye Summary link to view current and historical patient findings in a grid. Expand the General panel to review general patient information, the Health Monitor, and Internal Physician Notes. Expand the Orders panel to review, update, and add orders. Expand the Medications panel to review or update medications and perform medication reconciliation, if appropriate. Expand the Reason for Visit panel to review the technician conversation, makes any updates, and sign-off on

Document General Patient Information
In the General panel, select New patient or Established patient. The patient type is required for billing purposes and determines the type of visit code calculated on the Finalize tab. If the patient has been seen previously, the system automatically selects Established patient. Click the Historian field and select a value from the list or select the top blank line to type a different value. To enter a different date for First seen, click the field and select a date using the Calendar. For Vision Correction, select Yes or No. If yes, enter the Type and Years for the following: Glasses Contact Lenses Select OD or OS for Dominant Eye.
