Contagion Risk

The Contagion Risk link is available on every main Ophthalmic template. The Contagion Risk link and the corresponding badge count are used to indicate when the patient is at risk of being exposed to contagions. The Contagion Risk link opens the Contagion Exposure template. The Contagion Exposure template is used to document patient exposure to contagions and any travel to areas with a high risk of exposure to contagions. You can view historical information for risk of exposures to contagions and an exposure log for the patient in the corresponding grids. You can add, update, remove, or deactivate contagion exposure information from the Contagion Exposure History grid when working on an unlocked encounter. When you enter a contagion on the Contagions Exposure template, the contagion is automatically flagged and remains in the badge count until it is deactivated. Begin a New Ophthalmic Encounter

When a patient arrives, intake staff can open the patient chart and the Home Page - OPH template to begin documenting the new encounter for Ophthalmology and Optometry. You can review historical patient data and update allergies and medications. Open the Home Page - OPHtemplate. The template opens with the specialty and visit type defaulted to Ophthalmology and Office Visit. To change the specialty, click the Specialty field, and select a value from the list. To change the visit type, click the Visit Type field, and select a value from the list. Review the information in the following panels: General- review or update the general patient information Health Monitor- review or update the dates of the most recent testing and procedures Historic HPI Impression/Plan Summary- view the history of the patient's impression or plan information Visual Acuity- view the pateint's visual acuity measurements from previous encounters Orders- view and manage orders IOP- view the pateint's intraocular p

Document Risk Factor Indicators

Click the Configure icon on the top corner of the template.The Oph Risk Factors Config template opens. oph risk factors config If the patient has no risk conditions, select the No risk indicators check box and then click Save & Close. Select Yes, No, or Unknown for the following risk factors. Diabetes Diabetic Retinopathy Hypertension Macular Degeneration Glaucoma To document tobacco use, click the Tobacco Usage link and enter tobacco usage information. If appropriate, select the Tobacco cessation discussed check box. The Tobacco Cessation Discussed template opens for you to select additional details. Click Save & Close. Viewing Risk Factors

You can view details about patients' risk factors to help you to evaluate their health condition. The risk indicators for Tobacco use (TOB), Hypertension (HTN), Diabetes (DM), and Coronary artery disease (CAD) are available for documentation in NextGen® Adaptive Content Engine. Risk factors You can use the Risk Factors Config template to indicate a patient's current health conditions. You cannot consider a patient's health at high risk by looking at these conditions or you can view the risk indicator icons without opening the template. If you want to indicate that the patient is at high risk for developing a certain condition, you can document such details on the Care Guidelines template. You can use the risk indicators to review the risk factors without opening the Risk Factors Config template. Document the Risk Factors You can document the risk factors for patients. Risk Factors

You can document the risk factors for tobacco use (TOB), hypertension (HTN), diabetes (DM), and coronary artery disease (CAD) on the Risk Factors Config template. These are not the conditions that the patient is considered to be at high risk for. If you want to indicate that the patient is at high risk for developing a certain condition, you can document such information on the Care Guidelines template. A Risk Factors dashboard displays risk indicators for Tobacco use (TOB), Hypertension (HTN), Diabetes (DM), and Coronary artery disease (CAD) on every template in an open chart. The Risk Factors Config template is used to indicate conditions that the patient already has - not conditions that are considered to be high risk for the patient.
