Viewing Risk Factors

You can view details about patients' risk factors to help you to evaluate their health condition. The risk indicators for Tobacco use (TOB), Hypertension (HTN), Diabetes (DM), and Coronary artery disease (CAD) are available for documentation in NextGen® Adaptive Content Engine. Risk factors You can use the Risk Factors Config template to indicate a patient's current health conditions. You cannot consider a patient's health at high risk by looking at these conditions or you can view the risk indicator icons without opening the template. If you want to indicate that the patient is at high risk for developing a certain condition, you can document such details on the Care Guidelines template. You can use the risk indicators to review the risk factors without opening the Risk Factors Config template. Document the Risk Factors You can document the risk factors for patients. Chronic Conditions Data

The Chronic Conditions and Chronic Conditions HPI templates are used for documentation of multiple chronic conditions in one place, including diabetes. In addition, you can review health maintenance items, order tests, and review recommended care items and patient goals and/or self-management. Note: The Chronic Conditions and Chronic Conditions HPI templates cannot be used on the same encounter. To use the Chronic Conditions template, be sure that chronic conditions is not selected in the Reason for Visit panel. Risk Factors

You can document the risk factors for tobacco use (TOB), hypertension (HTN), diabetes (DM), and coronary artery disease (CAD) on the Risk Factors Config template. These are not the conditions that the patient is considered to be at high risk for. If you want to indicate that the patient is at high risk for developing a certain condition, you can document such information on the Care Guidelines template. Document the Risk Factors

You can document the risk factors for patients. Open NextGen® Enterprise EHR. On a patient encounter, open any of the standard visit templates. (for example, the Intake template. On the template header, select Config. The Risk Factors Config template opens. If the patient has no risk factors, select No risk indicators, and select Save & Close. If the patient has a history of tobacco use, do the following: In the Tobacco section, select Tobacco Usage.The Social History - Tobacco template opens. Note: If the tobacco information is updated on the Screening Summary template or on the Risk Factors Config template, the application automatically fills values on the Social History - Tobacco template. Enter data on the Social History - Tobacco template, and select OK. Select the Tobacco Cessation Discussed link.The Tobacco Cessation Discussed template opens. Select the Risk Factors link. The Risk Factors Config template opens. Risk Factors Config template Select the No risk indicators checkbox if the patient has no risk conditions, and then select Save & Close. For patients who have one or more of the risk conditions, do the following: In the Tobacco section, select Tobacco Usage to open the Social History - Tobacco template. Note: If the tobacco information is updated on the Screening Summary template or on the Risk Factors Config template, the application fills values in the Social History - Tobacco template. Document the applicable data on the template. Select the Tobacco Cessation Discussed link. The Tobacco Cessation Discussed template opens. Document the tobacco cessation discussion, and then select Save and Close. For more information on tobacco cessation counseling, select . Select Save & Close.
