Risk Factors

A Risk Factors dashboard displays risk indicators for Tobacco use (TOB), Hypertension (HTN), Diabetes (DM), and Coronary artery disease (CAD) on every template in an open chart. The Risk Factors Config template is used to indicate conditions that the patient already has - not conditions that are considered to be high risk for the patient. Providers or clinical staff members must manually enter this information to provide a quick visual cue to all staff members. Document the Risk Factors You can document the risk factors for patients. Risk Levels

You can view risk levels for a patient in color-coded columns by using the Risk-Stratification template. On the template header of every template, you can use the Risk Level link which open the Risk-Stratification template. You can also view the selected risk level (1-6) and the associated level color. When Enterprise Chart is enabled, all practices can view the Risk Level link. To enable the Risk Level link on the NextGen® Adaptive Content Engine templates, you must select the Enable Risk Level Stratification checkbox in the Risk Stratified Levels section on the Practice Configuration practice template. The History link on the Risk-Stratification template displays a patient’s risk level history in data grids. You can add, update, remove, and deactivate patient details from the History grid only when 3an encounter is unlocked. The history grid shows all changes made to the risk level for the patient, sorted by date in reverse chronological order. You can remove history details from the

Risk Factors

You can document the risk factors for tobacco use (TOB), hypertension (HTN), diabetes (DM), and coronary artery disease (CAD) on the Risk Factors Config template. These are not the conditions that the patient is considered to be at high risk for. If you want to indicate that the patient is at high risk for developing a certain condition, you can document such information on the Care Guidelines template. Document the Risk Factors

You can document the risk factors for patients. Open NextGen® Enterprise EHR. On a patient encounter, open any of the standard visit templates. (for example, the Intake template. On the template header, select Config. The Risk Factors Config template opens. If the patient has no risk factors, select No risk indicators, and select Save & Close. If the patient has a history of tobacco use, do the following: In the Tobacco section, select Tobacco Usage.The Social History - Tobacco template opens. Note: If the tobacco information is updated on the Screening Summary template or on the Risk Factors Config template, the application automatically fills values on the Social History - Tobacco template. Enter data on the Social History - Tobacco template, and select OK. Select the Tobacco Cessation Discussed link.The Tobacco Cessation Discussed template opens. NextGen Healthcare recommends you to use some of the common features, such as Patient Information Card, panel controls, and links from the left navigation bar, that are available in a standard visit template set for best work outcomes. Patient Information Card You can view pertinent information about a patient through the Patient Information Card (PIC). Template Headers for Displaying Patient Information You can review template headers for patient and change the display.
