Viewing Risk Factors

You can view details about patients' risk factors to help you to evaluate their health condition. The risk indicators for Tobacco use (TOB), Hypertension (HTN), Diabetes (DM), and Coronary artery disease (CAD) are available for documentation in NextGen® Adaptive Content Engine. Risk factors You can use the Risk Factors Config template to indicate a patient's current health conditions. You cannot consider a patient's health at high risk by looking at these conditions or you can view the risk indicator icons without opening the template. If you want to indicate that the patient is at high risk for developing a certain condition, you can document such details on the Care Guidelines template. You can use the risk indicators to review the risk factors without opening the Risk Factors Config template. Document the Risk Factors You can document the risk factors for patients. Risk Factor Indicators

To ensure that patient health risk factors are not overlooked, the Derm Risk Factors Config link is available on the Dermatology specialty templates. The link indicates whether the patient has risk factors or not. The following indicators are available: Indicators Description No risk factor The patient does not have the health risk factor. risk indicator unknown It is not known if the patient has the health risk factor. risk indicator yes The patient has the health risk factor. Risk Factors

You can document the risk factors for tobacco use (TOB), hypertension (HTN), diabetes (DM), and coronary artery disease (CAD) on the Risk Factors Config template. These are not the conditions that the patient is considered to be at high risk for. If you want to indicate that the patient is at high risk for developing a certain condition, you can document such information on the Care Guidelines template. A Risk Factors dashboard displays risk indicators for Tobacco use (TOB), Hypertension (HTN), Diabetes (DM), and Coronary artery disease (CAD) on every template in an open chart. The Risk Factors Config template is used to indicate conditions that the patient already has - not conditions that are considered to be high risk for the patient. Providers or clinical staff members must manually enter this information to provide a quick visual cue to all staff members. Document the Risk Factors You can document the risk factors for patients. Document Risk Factor Indicators

Click the Configure icon on the top corner of the template.The Oph Risk Factors Config template opens. oph risk factors config If the patient has no risk conditions, select the No risk indicators check box and then click Save & Close. Select Yes, No, or Unknown for the following risk factors. Diabetes Diabetic Retinopathy Hypertension Macular Degeneration Glaucoma To document tobacco use, click the Tobacco Usage link and enter tobacco usage information. If appropriate, select the Tobacco cessation discussed check box. The Tobacco Cessation Discussed template opens for you to select additional details. Click Save & Close.
