Record the Patient's Derm Risk Factors

Open NextGen® Enterprise EHR and select a Established patient or New patient as appropriate. Create a patient encounter and open the Intake template. Select a Specialty of Dermatology and a Visit Type of Office Visit.The Derm Risk Factors Config link is available on the patient header of every dermatology template Click the Config icon. The Derm Risk Factors Config template opens. If the patient does not have any risk factor, select the No risk indicators check box. In the Tobacco section, document the smoking status of the patient as follows: Click Tobacco Usage to document the tobacco consumption details such as tobacco type and usage per day on the Social - History Tobacco template. Click Tobacco cessation discussed to document smoking cessation efforts and passive smoke exposure on the Tobacco Cessation Discussed template. If your practice provides treatment for substance use disorder, you must include the long form or short form of disclosure for Substance Abuse and Mental Health Services Administration (SAMHSA) 42 CFR Part 2 provisions in the documents and reports that you generate. You can identify a patient's risk factors by viewing the risk indicators associated with certain health conditions. The following table shows the risk indicators that appear for hypertension when the corresponding options are selected on Risk Factors Config. Document Risk Factor Indicators

Click the Configure icon on the top corner of the template.The Oph Risk Factors Config template opens. oph risk factors config If the patient has no risk conditions, select the No risk indicators check box and then click Save & Close. Viewing Risk Factors

You can view details about patients' risk factors to help you to evaluate their health condition. The risk indicators for Tobacco use (TOB), Hypertension (HTN), Diabetes (DM), and Coronary artery disease (CAD) are available for documentation in NextGen® Adaptive Content Engine. Risk factors You can use the Risk Factors Config template to indicate a patient's current health conditions. You cannot consider a patient's health at high risk by looking at these conditions or you can view the risk indicator icons without opening the template. If you want to indicate that the patient is at high risk for developing a certain condition, you can document such details on the Care Guidelines template. You can use the risk indicators to review the risk factors without opening the Risk Factors Config template. Document the Risk Factors You can document the risk factors for patients.
