Patient's Tobacco Use Information

You can document the patient's current or past tobacco use information. You can document the patient's tobacco cessation efforts, vaping use details, and exposure to passive tobacco smoke in the following panels on the Social History - Tobacco template. Tobacco Use Historical Use Efforts To Quit Tobacco Vaping Use Passive Exposure Social History - Tobacco You can open the Social History – Tobacco template through the navigation bar, the History Summary panel on the Intake template, the Social panel on the Histories template, or the Risk Factors Config template. Document Patient's Current Tobacco Use Details You can view and document the current tobacco use details for a patient. View Tobacco Use History You can view the usage history of tobacco and smokeless tobacco products. Add Tobacco and Vaping Details

You can document the current tobacco use details and vaping details of a patient. To open the Social History – Tobacco template, do one of the following: On the Histories template, expand the Social panel, select Tobacco/Vaping Use, and click Add/Edit. In the History Summary panel on the Intake template, click Social, click Tobacco/Vaping Use, and then click Add/Edit. Social History – Tobacco Expand the Tobacco Use panel. Note: For a patient whose age is 12 and above, the smoking status must be recorded to meet the Quality Reporting requirements. Select the appropriate options for the questions, and document the details in the necessary sections according to the tobacco usage by the patient. Document vaping details in the Vaping Use panel and enter any comments. Document the Social History of the Patient

You can document the social history of a patient from the Social panel on the Histories template. Before you begin You must clear the Enable Panel version of Adult Social History check box on the Practice Configuration template to open the Social History – Tobacco template. Open NextGen® Enterprise EHR. In the Social panel on the Histories template, select Tobacco/Vaping Use link from the left pane, and select Add/Edit. The Social History - Tobacco template opens. Note: You can select another link from the left pane to open the associated template. Depending on your specialty configurations, you can also open the Social History – Tobacco template from the navigation bar. Document the Reason for Not Screening a Patient for Vaping

You can document the reason for not screening a patient for vaping in using the Social History – Tobacco template. Open the Histories template. In the Social panel, click Add. The Social History – Tobacco template opens. Expand Vaping Use. Note: The fields in this panel are unavailable until you select the Yes option for Screened for vaping. For Screened for vaping, select No. In Vaping comments, enter the reason for not screening the patient for vaping. Document Passive Exposure

Recording passive smoke exposure is not a Quality Reporting requirement, nor does it record a patient’s smoking status in Quality Reporting. Open NextGen® Enterprise EHR. To open the Social History – Tobacco template, do one of the following: On the Histories template, expand the Social panel, and select Add/Edit. In the History Summary panel on the Intake template, select Social, select Tobacco/Vaping Use, and select Add/Edit. The Social History – Tobacco template opens. Expand the Passive Exposure panel. In Have you ever had passive smoke exposure, select the applicable option. If you select No/Never, go to step 10. Otherwise, go to step 5. In Exposure in home environment, select Yes or No. In Other exposure locations, add other locations of passive exposure. In Tobacco type, select all tobacco types the patient is exposed to.
