Patient's Adult Social History

You can view and document the social history of an adult patient. You can document the social history information of the patient in a one-page panel format on the Adult Social History template. This template does not appear by default. The Social History Review section allows you to document the review status. You can select an option to complete the social history review. You can select the Enable Panel version of Adult Social History checkbox in the Social History section of the Practice Configuration practice template to set the template as default. Patient's Tobacco Use Information

You can document the patient's current or past tobacco use information. You can document the patient's tobacco cessation efforts, vaping use details, and exposure to passive tobacco smoke in the following panels on the Social History - Tobacco template. Document the History Review

You can document the review details of a patient's history in the generated encounter document. You can also document the review status of medical, surgical, family, and social histories on the template. Open NextGen® Enterprise EHR. On the Histories template, select the History Review link.The History Review template opens. In Med/Surg/Interim Hx, Family, and Social, select the applicable option. Detailed document allows you to view all the details entered in the Histories template in the encounter document. Reviewed, updated allows you to indicate that you have reviewed and updated the patient's histories. Reviewed, no changes allows you to indicate that you have reviewed the patient's histories but made no changes. History unobtainable allows you to indicate that the information is not obtainable and enter the reason in the corresponding field. Select Save & Close. Note: Each reviewed area (Med/Surg, Family, and Social Histories) contributes to the Evaluation and Management coding.

Document Patient's Current Tobacco Use Details

You can view and document the current tobacco use details for a patient. Open NextGen® Enterprise EHR. To open the Social History – Tobacco template, do one of the following: On the Histories template, on the Social panel, select Tobacco/Vaping Use, and select Add/Edit. In the History Summary panel on Intake, select Tobacco/Vaping Use from Social, and select Add/Edit. The Social History – Tobacco template opens. Expand the Tobacco Use panel. Note: For the current encounter, NextGen® Enterprise EHR automatically selects the tobacco use option you selected for the previous encounter. For Have you ever used tobacco?, select No/never, Yes, or Unknown. If you select No/Never, then continue with Document Passive Exposure. Note: When you select No\Never for Have you ever used tobacco? then the system automatically selects No for the next two screening questions. Document Vaping Details of a Patient

Open NextGen® Enterprise EHR. Open the Histories template. In the Social panel, select Tobacco/Vaping Use and select Add/Edit.The Social History – Tobacco template opens. Expand the Vaping Use panel. In the Screened for vaping field, select Yes. Select a Status for vaping. If the patient is a current user, select a number in the Age started field. If the patient is not a current user, select a number in the Age started and Age stopped fields. Select any of the following fields and select an appropriate option from the corresponding list: Device type Frequency Current strengthNote: This is used to select the current nicotine percentage. If you select 0%, the Vaping without Nicotine checkbox is selected automatically. Document Tobacco Cessation Efforts

You can document a patient's tobacco cessation efforts in the Efforts to Quit Tobacco panel on the Social History – Tobacco template. Open NextGen® Enterprise EHR. To open the Social History – Tobacco template, do one of the following: On the Histories template, expand the Social panel, and select Add/Edit. In the History Summary panel on the Intake template, select Social, select Tobacco/Vaping Use, and select Add/Edit. On the Social History – Tobacco template, expand the Efforts to Quit Tobacco panel. Select the Have you ever tried to quit using tobacco? option. If you select No/never, document details of the discussions on tobacco cessation by selecting the Tobacco cessation discussed link. Social History – Tobacco To add the details of the patient's efforts to quit tobacco, do the following: Select the tobacco type, and then enter the date the patient attempted to quit tobacco.
