Add Social History

Before you begin You can configure the show confidential history, panel version of adult and pediatric social history templates from the Social History section of the Practice Configuration template. For more information about these settings, go to NextGen Healthcare Success Community and download the Practice Configuration Template User Guide for NextGen® Adaptive Content Engine You can add social history information in the History Summary section. The Social section uses age-specific templates based on the patient age and the Pediatric Age configuration on the Practice Configuration practice template. Open NextGen® Enterprise EHR. Open the Intake template. In the History Summary section, select Social. Select Select Social Hx to Add/Edit and select the relevant option from the list.Based on the patient age, the relevant social history template opens. Document the reported history information. Document the Patient's Social History on Tobacco Usage

You can document a patient's social history on tobacco usage in the Social History - Tobacco template. Before you begin You must clear the Enable Panel version of Adult Social History checkbox on the Practice Configuration template to open the Social History – Tobacco template. Open NextGen® Enterprise EHR. Open the Histories template, expand the Social panel, select the Tobacco/Vaping Use link from the left pane, and then select Add/Edit.The Social History - Tobacco template opens. Note: Depending on your specialty's configurations, you can also open the Social History – Tobacco template from the navigation bar. Document the necessary details in the following panels: Tobacco Use Historical Use Efforts To Quit Tobacco Vaping Use Passive Exposure Enter any additional comments, and select OK. Add Environmental Information that Affect the Patient You can add residence, animal and miscellaneous details about the environmental factors that affect the patient on the Environmental template. M

Patient's Social History

You can document a patient's social history. You can open the individual social history template or the Adult Social History template to document the social history information of a patient. The Adult Social History template includes the existing social history templates arranged in a one-page panel format. Each panel includes the data fields from the respective social history template. You can choose to document the social history information of the patient on individual social history templates or on the Adult Social History template. You can document the following social information for an adult patient on the social history templates or the Adult Social History template: Tobacco/Vaping Use Alcohol/Caffeine Statuses Lifestyle Occupation Comment Diet History Environmental Note: NextGen® Adaptive Content Engine is packaged with the individual social history templates as the default social history templates. Patient's Tobacco Use Information

You can document the patient's current or past tobacco use information. You can document the patient's tobacco cessation efforts, vaping use details, and exposure to passive tobacco smoke in the following panels on the Social History - Tobacco template. Enter Tobacco Usage

The client's current smoking status must be recorded to meet Quality Reporting requirements. The Social History - Tobacco template in the Behavioral Health template set only includes the Tobacco option as the other social history data is documented in different sections of the template set. This ensures that users are documenting the required data and prevents duplicate information from printing in the documents. In the Substance Use/Addictive Behavior History panel, click the Tobacco Usage link. Select one of the following options for Have you ever used tobacco?: No/never Yes Unknown If you select No/never or Unknown, click Save & Close to return to your previous workflow. Select Yes or No for Have you used tobacco in the last 30 days? Select Yes or No for Have you used smokeless tobacco products in the last 30 days? You can view and document the social history of an adult patient. You can document the social history information of the patient in a one-page panel format on the Adult Social History template. This template does not appear by default. Adult Social History The Social History Review section enables you to document the review status. You can select an option to complete the social history review. You can select the Enable Panel version of Adult Social History check box in the Social History section of the Practice Configuration practice template to set the template as default.
