Document the Patient's Social History on Tobacco Usage

You can document a patient's social history on tobacco usage in the Social History - Tobacco template. Before you begin You must clear the Enable Panel version of Adult Social History checkbox on the Practice Configuration template to open the Social History – Tobacco template. Open NextGen® Enterprise EHR. Open the Histories template, expand the Social panel, select the Tobacco/Vaping Use link from the left pane, and then select Add/Edit.The Social History - Tobacco template opens. Note: Depending on your specialty's configurations, you can also open the Social History – Tobacco template from the navigation bar. Document the necessary details in the following panels: Tobacco Use Historical Use Efforts To Quit Tobacco Vaping Use Passive Exposure Enter any additional comments, and select OK. Add Environmental Information that Affect the Patient You can add residence, animal and miscellaneous details about the environmental factors that affect the patient on the Environmental template. M

Patient's Tobacco Use Information

You can document the patient's current or past tobacco use information. You can document the patient's tobacco cessation efforts, vaping use details, and exposure to passive tobacco smoke in the following panels on the Social History - Tobacco template. Tobacco Use Historical Use Efforts To Quit Tobacco Vaping Use Passive Exposure Social History - Tobacco You can open the Social History – Tobacco template through the navigation bar, the History Summary panel on the Intake template, the Social panel on the Histories template, or the Risk Factors Config template. Document the History Review

You can document the review details of a patient's history in the generated encounter document. You can also document the review status of medical, surgical, family, and social histories on the template. Open NextGen® Enterprise EHR. On the Histories template, select the History Review link.The History Review template opens. In Med/Surg/Interim Hx, Family, and Social, select the applicable option. Detailed document allows you to view all the details entered in the Histories template in the encounter document. Reviewed, updated allows you to indicate that you have reviewed and updated the patient's histories. Reviewed, no changes allows you to indicate that you have reviewed the patient's histories but made no changes. History unobtainable allows you to indicate that the information is not obtainable and enter the reason in the corresponding field. Select Save & Close. Note: Each reviewed area (Med/Surg, Family, and Social Histories) contributes to the Evaluation and Management coding.

Document Tobacco Cessation Efforts

You can document a patient's tobacco cessation efforts in the Efforts to Quit Tobacco panel on the Social History – Tobacco template. Open NextGen® Enterprise EHR. To open the Social History – Tobacco template, do one of the following: On the Histories template, expand the Social panel, and select Add/Edit. In the History Summary panel on the Intake template, select Social, select Tobacco/Vaping Use, and select Add/Edit. On the Social History – Tobacco template, expand the Efforts to Quit Tobacco panel. Select the Have you ever tried to quit using tobacco? option. If you select No/never, document details of the discussions on tobacco cessation by selecting the Tobacco cessation discussed link. Social History – Tobacco To add the details of the patient's efforts to quit tobacco, do the following: Select the tobacco type, and then enter the date the patient attempted to quit tobacco. Enter the longest time that the patient was tobacco free. You can enter number of days, weeks, months, or

Patient's Adult Social History

You can view and document the social history of an adult patient. You can document the social history information of the patient in a one-page panel format on the Adult Social History template. This template does not appear by default. The Social History Review section allows you to document the review status. You can select an option to complete the social history review. You can select the Enable Panel version of Adult Social History checkbox in the Social History section of the Practice Configuration practice template to set the template as default. You can document a patient's social history. You can open the individual social history template or the Adult Social History template to document the social history information of a patient. The Adult Social History template includes the existing social history templates arranged in a one-page panel format. Each panel includes the data fields from the respective social history template. You can choose to document the social history information of the patient on individual social history templates or on the Adult Social History template.
