You can record the patient's status using the Social History - Status template.
 
  - Open NextGen® Enterprise EHR.
 
  - Open the Histories template.
 
  - In the Social panel, select Statuses, and then select Add/Edit.
   
    The Adult Social History template opens.
   
 
  - Expand the Statuses panel.
 
  - In the Demographics section, enter the patient details.
 
  - Select the Employment Information link, and on the Employment/Occupation template, select Add to create a new record.
 
  - Select Save, and then select Close.
 
  - Select the occupational hazards or enter the hazards manually.
   
    You can enter up to two hazards.
   
 
  - In the Radiation Exposure section, enter the exposure date, radiation type, and the radiation dose and unit.
   
    By default, the encounter date appears as the exposure date. You can select another date by selecting Cl.
   
 
  - To view the radiation exposure history, select the Exposure History link.
 
  - In the Military section, indicate whether the patient has military experience, and then do one of the following:
   
    - If you select Yes, enter the current status, military branch, years served, and type of discharge.
 
    - Select the appropriate option for Noise exposure, Biohazard exposure, and Stationed Overseas.
 
   
 
  - In the Education section, enter the patient's education, degree, and the country the degree was obtained from, and do the following:
   
    - To document additional educational details, select the Details link.
 
    - On the Education Detail template, enter the school name, school type, and grade.
 
    - Select the appropriate option for Learning disability, Special needs, and Gifted Program.
 
    - If the patient has repeated any grades, select Yes, and enter the grade and reason.
 
    - Indicate whether the patient is suspended or expelled from school, and enter the details.
 
    - Select Save & Close.
 
   
 
  - Enter the details in the Marital Status/Family/Social Support section.
 
  - Select OK.
