Add Education and Leisure Activities Details

You can document the details of education and leisure activities of a patient on the Pediatric Social History – Education template.

1. Open NextGen® Enterprise EHR.
2. To open the Pediatric Social History – Education template, do one of the following:
   • On the Histories template, expand the Social panel, select Education, and then click Add/Edit.
   • In the History Summary panel of the Intake template, click Social, click Education, and then click Add/Edit.
     

3. In the Historian field, enter the relationship of the person who provides the patient information.
4. In the Education section, do the following:
   1. Enter the patient's current school name and grade.
   2. Indicate if the patient has repeated any grades in school.
      • If you select Yes, then enter the grade and the reason for the repetition.
   3. Select the grades the patient received in school.
   4. Select the appropriate options for the following:
      • Performing
      • Likes school
      • Truancy
      • Suspended or expelled
        • If you select Yes, then enter the reason.
      • Learning disability
        • If you select Yes, then enter the type of disorder.
      • Special needs
        • If you select Yes, then select a value.
      • Gifted program
      • College prep
      • High school graduate
5. In the Sleep section, select the appropriate options.
   • If you select Yes, click the Detail button to enter additional details, and then click Save & Close.
6. In the Activity section, click the following fields, and select the number of hours spent on each activity:
   • Exercise/sports
   • TV/computer games
   • Internet
7. In the Recent Travel section, do the following:
   1. Select the appropriate checkboxes to indicate the recent travel history.
      The Where, Return date, and Duration fields appear for the selected checkboxes.

      • Enter the travel location, return date, and travel duration.
   2. Select Travel exposure and the appropriate risk, or enter the data manually in the fields.
8. To enter any additional details on the patient's education, click the Comments link.
9. Save and close the template.
