Document Pediatric Alcohol History
You can document the alcohol history of a pediatric patient.
- Open NextGen® Enterprise EHR.
 - To open the Eds Confidential History - Substances template, do one of the following:
   
- In the Social panel on the Histories template, click the Confidential History link.
 - In the History Summary panel on the Intake template, click the Confidential link.
 
 - If the consent has been obtained to print the confidential details, select the Include confidential information in the pediatric Social History document with patient/guardian permission checkbox.
 - In the Alcohol Use section, indicate if the patient drinks alcohol. 
   
- If you select Yes or Formerly, then enter the type of alcohol, frequency of alcohol consumption, amount, and the details of the last drink.
 
 - Indicate if the patient has sought a treatment for alcohol abuse.
   
- If you select Yes, then enter the date of last treatment for alcohol abuse and the number of treatments taken for alcohol abuse.
 
 - Indicate if the patient had withdrawal problems, seizures, or blackouts from alcohol or drugs.
 - Indicate if the patient has been involved in a 12-step program.
   
- If you select Yes, then select a checkbox to indicate if the patient is currently involved or was previously involved.
 
 - Indicate if the patient had taken emergency medical attention due to intoxication.
   
- If you select Yes, then enter the number of emergency treatments taken for intoxication.
 
 - Indicate if there is any family history of alcoholism. If you select Yes, do the following:
   
- Select a family member in the list. 
      Note: To select multiple family members, click the other three fields.
 
 - Select a family member in the list. 
      
 - Enter the number of times in the past year the patient had taken five or more drinks in a day.
 - Click Save & Close.
