Document Pediatric Psych History

You can document the pediatric psych history details.

1. Open NextGen® Enterprise EHR.
2. To open the confidential templates, do one of the following:
   • In the Social panel on the Histories template, click the Confidential History link.
   • In the History Summary panel on the Intake template, click the Confidential link.
   The Peds Confidential History - Substances template opens displaying the links to the confidential templates in the left pane.
3. Click Psychiatric/Placement.
   The Peds Confidential History - Psychiatric template opens.
4. If someone other than the patient is providing the details as a historian, then enter the historian details in Historian.
5. Indicate the history of suicidal and homicidal thoughts.
6. Indicate if the child has been diagnosed with a psychiatric problem.
   • If you select Yes, then enter up to two diagnoses, onset age, and the patient treatment.
7. Enter the name of the psychiatrist or therapist, and then type their phone numbers in the corresponding fields.
8. Indicate the consent status for communication with PCP.
9. Indicate the family history of psychiatric problems.
   • If you select Yes, then select the psychiatric problem and indicate the patient's relation with the family member.
10. To add the placement history details, enter the details in the Placement History section.
11. To add the incarceration details, enter the details in the Incarceration section.
12. Click Save & Close.
