You can use the confidential history templates to document a patient's confidential details.
You can open the
Peds Confidential History template by clicking the following links:
- The Confidential History link in the Social panel on the Histories template.
- The Confidential link in the History Summary panel on the Intake template.
You can document the following confidential details of the pediatric patient:
- Alcohol and drug use, and any related comments from the historian on the Peds Confidential History - Substances template.
- Psychiatric history, child placement history, and incarceration details from the historian on the Peds Confidential History - Psychiatric template.
- Sexual practices, Sexually Transmitted Infection (STI) details, and any related comments from the historian or the provider, or from both on the Peds Confidential History - Sexual Hx template.
- Child abuse history and any related comments from the historian on the Peds Confidential History - Abuse template.
With the patient's permission, you can include the documented confidential details and generate the social history document. To include the details in the social history document, you need to select the Include confidential information in the pediatric Social History document with patient/guardian permission checkbox on the Peds Confidential History - Substances template.
