Confidential Information

You can use the confidential history templates to document a patient's confidential information. You can open the confidential history templates by selecting the following links. The Confidential History link in the Social panel on the Histories template. The Confidential link in the History Summary panel on the Intake template. The links allows you to document the following confidential information of the patient. Alcohol and drug use On the Confidential - Alcohol/Drug template. Psychiatric history On the Confidential - Psychiatric template. Abuse or domestic violence and incarceration On the Confidential - Abuse/Incarceration template. Sexual practices and STI details On the Confidential - Sexual Practices/STIs template. With the patient's permission, you can generate the documented confidential details in the social history document. You need to select the Include all confidential information in the social history document with patient permission checkbox on the templates to include

Patient's Adult Social History

You can view and document the social history of an adult patient. You can document the social history information of the patient in a one-page panel format on the Adult Social History template. This template does not appear by default. The Social History Review section allows you to document the review status. You can select an option to complete the social history review. You can select the Enable Panel version of Adult Social History checkbox in the Social History section of the Practice Configuration practice template to set the template as default. Social Confidential History

The Social Confidential history configured in the Practice Configuration and Document Configuration practice templates displays in the documents. The Confidential Social history can be configured to always display in the document in the Practice Configuration template. If the setting in the Practice Configuration template is not set to automatically display the confidential social history, then this information will not display in the document unless the checkbox is selected in the Confidential Social History template. Patient's Social History

You can document a patient's social history. You can open the individual social history template or the Adult Social History template to document the social history information of a patient. The Adult Social History template includes the existing social history templates arranged in a one-page panel format. Each panel includes the data fields from the respective social history template. You can choose to document the social history information of the patient on individual social history templates or on the Adult Social History template. Pediatric Confidential Information

You can use the confidential history templates to document a patient's confidential details. You can open the Peds Confidential History template by clicking the following links: The Confidential History link in the Social panel on the Histories template. The Confidential link in the History Summary panel on the Intake template. You can document the following confidential details of the pediatric patient: Alcohol and drug use, and any related comments from the historian on the Peds Confidential History - Substances template. Psychiatric history, child placement history, and incarceration details from the historian on the Peds Confidential History - Psychiatric template. Document Psychiatric Information

You can document the psychiatric health information of a patient on the Confidential – Psychiatric template. Open NextGen® Enterprise EHR. Open the Histories template. In the Social panel, select the Confidential History link. The Confidential - Alcohol/Drug template opens displaying the links to the confidential templates, in the left pane. Select the Psychiatric link. Confidential – Psychiatric The Confidential – Psychiatric template opens. Select Yes or No for History of suicidal thoughts, History of homicidal thoughts, and Diagnosed with psychiatric problem. If available, enter the name and phone number of the psychiatrist and therapist. Select Yes or No to indicate the patient's consent for communication between the psychiatrist and PCP, and the therapist and PCP. If you select Yes, select the Yes or No option to indicate if the documentation is on file. Select Yes or No to indicate if there is any family history of psychiatric problems.
