Add Environmental Information that Affect the Patient

Before you begin You must select the Enable Panel version of Adult Social History checkbox on the Practice Configuration template to open the Adult Social History template. Open NextGen® Enterprise EHR. In the Social panel, select Add/Edit. The Adult Social History template opens. In the Environmental panel, in the Residence section, do the following: To add information about the patient's primary residence, select 1 for Residence. Enter the appropriate residence information in the respective fields, select Add.The residence information appears in the grid. Repeat the steps to add a second residence for the patient using the 2 option for Residence. In the Animals section, do the following: To document animal information, select 1 or 2 for Residence. Enter the animal type and environmental factors, then select Add. In the Miscellaneous section, enter the miscellaneous information and select OK. Document the History Review

You can document the review details of a patient's history in the generated encounter document. You can also document the review status of medical, surgical, family, and social histories on the template. Open NextGen® Enterprise EHR. On the Histories template, select the History Review link.The History Review template opens. In Med/Surg/Interim Hx, Family, and Social, select the applicable option. Detailed document allows you to view all the details entered in the Histories template in the encounter document. Reviewed, updated allows you to indicate that you have reviewed and updated the patient's histories. Reviewed, no changes allows you to indicate that you have reviewed the patient's histories but made no changes. History unobtainable allows you to indicate that the information is not obtainable and enter the reason in the corresponding field. Select Save & Close. Note: Each reviewed area (Med/Surg, Family, and Social Histories) contributes to the Evaluation and Management coding.

Document the Patient's Social History on Tobacco Usage

You can document a patient's social history on tobacco usage in the Social History - Tobacco template. Before you begin You must clear the Enable Panel version of Adult Social History checkbox on the Practice Configuration template to open the Social History – Tobacco template. Open NextGen® Enterprise EHR. Open the Histories template, expand the Social panel, select the Tobacco/Vaping Use link from the left pane, and then select Add/Edit.The Social History - Tobacco template opens. Note: Depending on your specialty's configurations, you can also open the Social History – Tobacco template from the navigation bar. Document the necessary details in the following panels: Tobacco Use Historical Use Efforts To Quit Tobacco Vaping Use Passive Exposure Enter any additional comments, and select OK. Add the Patient's Employment Details

You can add the patient's employment or occupation details using the Social History - Employment template. Open NextGen® Enterprise EHR. To add the patient's employment status, do one of the following: On the Histories template, expand the Social panel, select Employment, and then select Add. In the History Summary panel on the Intake template, select Employment from the Social panel, and then select Add. The Adult Social History template opens. Expand the Employment panel, select Add.The "Employment/Occupation" - [New Record] template opens. Select the employer's name, and then select OK. Note: To update the employer's name, select Add/Update, and enter the details in the Modify Patient Information template. Enter the Occupation and Industry details. Enter the details for Occupational Hazards, and then select Restrictions from the list. Enter the hire and termination dates. Select the employment duration, and then enter the reason for leaving. Manage Patient History Records

You can update or remove the patient history records for medical, surgical, interim, social, and family categories. Open NextGen® Enterprise EHR. Open the Intake template, and then expand the History Summary section. Do one of the following: To update or remove a history record for the Past Medical/Surgical category on the relevant template, select Add Past Medical/Surgical Hx. To update or remove a history record for the Social category on the relevant template, select Select Social Hx to Add/Edit, and then select the appropriate option from the list. To update or remove a history record for the Interim and Family categories on relevant templates, select the record from the grid, right-click, and then select Open. Add Home Environment and Safety Measures Details

You can document the home environment and safety measure details of a pediatric patient. Open NextGen® Enterprise EHR. To open the Pediatric Social Hx – Home Environment template, do one of the following: On the Histories template, expand the Social panel, select Home Environment, and then click Add/Edit. In the History Summary panel on the Intake template, click Social, click Environmental, and then click Add/Edit. Pediatric Social Hx – Home Environment In the Historian field, enter the relationship of the person who provides the patient information. Select the appropriate option for hand dominance. To add or update the preferred language, click the Preferred language field to open the Modify Patient Information template and add the preferred language.
