Historical Visual Acuity Measurements

The Visual Acuity grid on the Home Page - OPH template displays the patient's visual acuity (VA) measurements from previous encounters. Begin a New Ophthalmic Encounter

When a patient arrives, intake staff can open the patient chart and the Home Page - OPH template to begin documenting the new encounter for Ophthalmology and Optometry. You can review historical patient data and update allergies and medications. Open the Home Page - OPHtemplate. The template opens with the specialty and visit type defaulted to Ophthalmology and Office Visit. To change the specialty, click the Specialty field, and select a value from the list. To change the visit type, click the Visit Type field, and select a value from the list. Review the information in the following panels: General- review or update the general patient information Health Monitor- review or update the dates of the most recent testing and procedures Historic HPI Impression/Plan Summary- view the history of the patient's impression or plan information Visual Acuity- view the pateint's visual acuity measurements from previous encounters Orders- view and manage orders IOP- view the pateint's intraocular p

On the Prelim Exam - OPH and Provider - OPH templates, the IOP section displays current encounter exam findings, as well as findings for the last five encounters. iop panel data On the Oph EyeTech template, the IOP panel is used to document exam details and findings and displays historical exam findings in the grid. IOP measurements can be entered in mm Hg and/or Kilopascals (kPa). Document IOP Findings

Expand the IOP panel. The Historical IOP data appears in the grid. To track the reduction between pre-treatment IOP and the IOP during or after treatment, starting with the current encounter: Click a Baseline IOP field and select an appropriate baseline pre-treatment value from the list or select the top blank line and type in a value. Enter a Baseline IOP value for OD and OS. Note: Baseline IOP values are entered one time. Click the Method field and select the method used for measuring IOP from the list or select the top blank line and type a value. To enter findings for intraocular pressure using mmHg. Click the OD field and select a value in the list. The list for OS opens automatically. Select a value for OS. Enter findings for intraocular pressure using kPa for OD and OS. The name of the current user fills the Taken by field. Type comments in the Comment field. If the patient was dilated while the measurements were taken, select the Dilated IOP check box.
