Patient History
You can document the patient's past ocular history, blood sugar control, and other ophthalmic history on the Histories - OPH template. Patient history types include the following:
- Past ocular history – includes ocular procedures for the patient
- Medical/Surgical/ Interim
- Family history- significant family history
- Social history- includes tobacco, alcohol, and caffeine use; general social history information, lifestyle, and occupational information
- Confidential History link- located in the Social History panel; Includes alcohol and drug information, psychiatric care information, abuse and /or incarceration, sexual practices, and sexually transmitted infections
- Glucose Monitoring – used to follow a patient’s history of blood sugar control and includes HbA1C and Glucose testing values by the patient or their physician
- History Review – found in history panels and opens the History Review template, which is used to document that the patient's history was reviewed or unobtainable, and to include detailed history information in the document generated at the end of the encounter
