Record Glucose and HbA1C Values

You can record glucose and hemoglobin A1C values reported in the Ophthalmic specialties on the Glucose Monitoring - OPH template. In the Glucose Monitoring panel, click Add. For Reported By, select Patient or Physician. Enter the Glucose value, record the date Last checked, and enter the Time. Enter the HbA1C value and record the date Last checked. Patient History

You can document the patient's past ocular history, blood sugar control, and other ophthalmic history on the Histories - OPH template. For more information about documenting a patient's history, go to NextGen Healthcare Success Community and download the latest Getting Started User Guide for NextGen® Adaptive Content Engine. Patient history types include the following: Past ocular history – includes ocular procedures for the patient Medical/Surgical/ Interim Family history- significant family history Social history- includes tobacco, alcohol, and caffeine use; general social history information, lifestyle, and occupational information Confidential History link- located in the Social History panel; Includes alcohol and drug information, psychiatric care information, abuse and /or incarceration, sexual practices, and sexually transmitted infections Glucose Monitoring – used to follow a patient’s history of blood sugar control and includes HbA1C and Glucose testing values by the patient o

General Panel for Providers

Document Lab Glucose Results

On the Chronic Conditions - Hpi template, click the Lab Review link. The Lab Review template displays external and in-house lab results. View or enter lab results data. Click Save & Close to return to the Chronic Conditions - Hpi template. Parent topic: Access the Chronic Conditions - Hpi Template

Document Patient Reported Glucose Results

On the Chronic Conditions - Hpi template, navigate to the Labs section. To document and graph the patient’s reported glucose results, click the Home Glucose Readings link. The Home Glucose Map template opens. To enter values, do one of the following actions: Click Download Meter Data. Click the Datefield to enter the client reported value and continue to enter data points in the resulting windows. To create a visual representation of the data, click the Graph Daily Readings and/or Graph Min., Max. & Average. Click Save & Close to return to the Chronic Conditions - Hpitemplate. Historical Visual Acuity Measurements

The Visual Acuity grid on the Home Page - OPH template displays the patient's visual acuity (VA) measurements from previous encounters.
