Glucose Monitoring

The Glucose Monitoring panel displays HbA1C and glucose test results reported by the patient and the physician. Document Patient Reported Glucose Results

On the Chronic Conditions - Hpi template, navigate to the Labs section. To document and graph the patient’s reported glucose results, click the Home Glucose Readings link. The Home Glucose Map template opens. To enter values, do one of the following actions: Click Download Meter Data. Click the Datefield to enter the client reported value and continue to enter data points in the resulting windows. To create a visual representation of the data, click the Graph Daily Readings and/or Graph Min., Max. & Average. Click Save & Close to return to the Chronic Conditions - Hpitemplate. Patient History

You can document the patient's past ocular history, blood sugar control, and other ophthalmic history on the Histories - OPH template. For more information about documenting a patient's history, go to NextGen Healthcare Success Community and download the latest Getting Started User Guide for NextGen® Adaptive Content Engine. Patient history types include the following: Past ocular history – includes ocular procedures for the patient Medical/Surgical/ Interim Family history- significant family history Social history- includes tobacco, alcohol, and caffeine use; general social history information, lifestyle, and occupational information Confidential History link- located in the Social History panel; Includes alcohol and drug information, psychiatric care information, abuse and /or incarceration, sexual practices, and sexually transmitted infections Glucose Monitoring – used to follow a patient’s history of blood sugar control and includes HbA1C and Glucose testing values by the patient o

Document General Patient Information

In the General panel, select New patient or Established patient. The patient type is required for billing purposes and determines the type of visit code calculated on the Finalize tab. If the patient has been seen previously, the system automatically selects Established patient. Click the Historian field and select a value from the list or select the top blank line to type a different value. To enter a different date for First seen, click the field and select a date using the Calendar. For Vision Correction, select Yes or No. If yes, enter the Type and Years for the following: Glasses Contact Lenses Select OD or OS for Dominant Eye. Enter HPI Data for Diabetes

The Diabetes Mellitus – HPI template enables you to document patient findings and blood glucose readings performed by a fingerstick. In the Reason for Visit panel, select diabetes. The Diabetes Mellitus - HPI template opens. To enter the fingerstick readings, in the Managed With section, select the Fingerstick blood sugars check box. The DM Fingerstick Readings template opens. Enter the new values, and then click OK. Enter additional HPI details as needed. Click Save & Close. Documented History of Present Illness (HPI) findings display the Reason for Visit panel. Access the Chronic Conditions - Hpi Template

You can access the Chronic Conditions - Hpi template by clicking the chronic conditions link in the Reason for Visit panel.
