You can access the list of saved NextGen Care® Population Management Hub reports on the Cohorts tab. The following reports are available: NextGen® Enterprise EHR memorized reports. NextGen® Enterprise PM memorized reports. NextGen® Enterprise EHR and NextGen® Enterprise PM report snapshots. You can sort the data available in Cohorts by selecting on the column header. You can view the patient clinical information in Care Opportunities, only if you have permission to access patient's data. Note: In NextGen Care® Population Management Hub, only patient level NextGen® Enterprise PM reports are available. Patient Grid

The patient grid displays the patient demographic details, financial and next appointment details. When you run the NextGen Care® Population Management Hub report, patient information appears in the patient grid. The patient grid displays the patient demographic details, patient's risk level (color-coded), patient portal enrollment status, financial information, and the next appointment details. Note: If multiple appointments are scheduled for a patient on the same day, the next immediate appointment of that day appears in the Next Appointment field when you run the report. When reports are filtered based on appointment schedule in the Appointment Book tab, the patient grid displays additional columns. Note: The following columns appear in the patient grid when they are enabled on the Grid + Data tab on the User Settings window.
