Cohorts

You can access the list of saved NextGen Care® Population Management Hub reports on the Cohorts tab. The following reports are available: NextGen® Enterprise EHR memorized reports. NextGen® Enterprise PM memorized reports. NextGen® Enterprise EHR and NextGen® Enterprise PM report snapshots. You can sort the data available in Cohorts by selecting on the column header. You can view the patient clinical information in Care Opportunities, only if you have permission to access patient's data. Note: In NextGen Care® Population Management Hub, only patient level NextGen® Enterprise PM reports are available. NextGen Care® Population Management Hub Reports

The NextGen Care® Population Management Hub reports displays a list of the individual patients identified as part of a predefined patient population. Cohorts Filter My Lists Schedule You can filter the patients based on the NextGen® Enterprise PM schedule data to support care management work-flows such as care team huddles and pre-visit planning. Patient Grid The patient grid displays the patient demographic details, financial and next appointment details. Care Opportunities Outreach Log Configuration of the User and Global Settings Configuration of the user and global settings in NextGen Care® Population Management Hub. Run a Report

You can run the NextGen Care® Population Management Hub report from Cohorts, My Lists, or Appointment Book. Access NextGen Care® Population Management Hub and do one of the following: From the Cohorts tab, or the My Lists tab expand the reports group that you want to run. From the Appointment Book tab, apply the filter options to display the patients. Select the report that you want to run and do one of the following: Select Run . Double-click the report. Report Snapshots in Population Management Hub

Reports can be created in NextGen® Enterprise PM using specific filters and qualifying criteria to identify a specific patient population, and then saved as a report snapshot. These report snapshots can be accessed in NextGen® Enterprise EHR from the Population Management Hub > Cohorts tab and used to generate a patient list for outreach purposes, such as sending messages via NextGen Care® Outreach or NextGen® PxP Portal. Viewing the NextGen® Enterprise PM Memorized Reports

You must ensure the following requirements are met to view the NextGen® Enterprise PM reports in Cohorts: You must have NextGen® Enterprise PM license to view the reports in Cohorts. You must have access permissions to view patient’s clinical data. If you don’t have access to patient’s clinical data as per the NextGen® Enterprise Exclusions list, then you cannot view the patient’s clinical data on Care Opportunities though NextGen® Enterprise PM reports and patient name appears as Restricted in patient grid. You must have access to view and run the NextGen® Enterprise PM reports. If you don’t have the access to run the NextGen® Enterprise PM reports, then you cannot run the reports available in NextGen Care® Population Management Hub. When you select any NextGen® Enterprise PM report on the Cohorts tab, patients’ information does not appear in patient grid. You must ensure patient level NextGen® Enterprise PM reports are created.
