Filter

You can view the criteria used to build the NextGen Care® Population Management Hub report on the Filter tab. Note: The Filter tab is available only for the NextGen® Enterprise EHR reports. The Filter Name column provides list of criteria used to build a report and the Filter Value column represents the SQL Server query equating to the criteria selected for the report. The NextGen Care® Population Management Hub reports displays a list of the individual patients identified as part of a predefined patient population. Cohorts Filter My Lists Schedule You can filter the patients based on the NextGen® Enterprise PM schedule data to support care management work-flows such as care team huddles and pre-visit planning. Patient Grid The patient grid displays the patient demographic details, financial and next appointment details. Run a Report

You can run the NextGen Care® Population Management Hub report from Cohorts, My Lists, or Appointment Book. Access NextGen Care® Population Management Hub and do one of the following: From the Cohorts tab, or the My Lists tab expand the reports group that you want to run. From the Appointment Book tab, apply the filter options to display the patients. Select the report that you want to run and do one of the following: Select Run . Double-click the report. Report Snapshots in Population Management Hub

Reports can be created in NextGen® Enterprise PM using specific filters and qualifying criteria to identify a specific patient population, and then saved as a report snapshot. These report snapshots can be accessed in NextGen® Enterprise EHR from the Population Management Hub > Cohorts tab and used to generate a patient list for outreach purposes, such as sending messages via NextGen Care® Outreach or NextGen® PxP Portal. NextGen® Enterprise PM and NextGen Enterprise Memorized Reports

After you set up a report with the filters that best fit your practice, you can save the memorized report settings so that they are available for use again. You do not have to run the report to memorize it. Memorizing a report saves the report parameters, not the actual data. Each time you run the memorized report, the report format remains the same but it always contains the current information from the database. NextGen® Enterprise PM memorizes date ranges in different ways, depending on the following criteria: If you use criteria like current month or month to date, then NextGen® Enterprise PM uses your server's current date to set the date range. This is referred to as a smart date. If you enter a custom date, then NextGen® Enterprise PM remembers that date and uses it to pull the report data. Both NextGen® Enterprise PM and NextGen® Enterprise EHR reports can be memorized. Once memorized, you can run a report from the list of memorized reports without the need to reconfigure the r

Viewing the NextGen® Enterprise PM Memorized Reports

You must ensure the following requirements are met to view the NextGen® Enterprise PM reports in Cohorts: You must have NextGen® Enterprise PM license to view the reports in Cohorts. You must have access permissions to view patient’s clinical data. If you don’t have access to patient’s clinical data as per the NextGen® Enterprise Exclusions list, then you cannot view the patient’s clinical data on Care Opportunities though NextGen® Enterprise PM reports and patient name appears as Restricted in patient grid. You must have access to view and run the NextGen® Enterprise PM reports. If you don’t have the access to run the NextGen® Enterprise PM reports, then you cannot run the reports available in NextGen Care® Population Management Hub. When you select any NextGen® Enterprise PM report on the Cohorts tab, patients’ information does not appear in patient grid. You must ensure patient level NextGen® Enterprise PM reports are created.
