Create a Snapshot Report

You can create snapshot of patient level NextGen® Enterprise PM reports and the NextGen® Enterprise reports. Set up the report by defining the columns and filters to produce the desired format and results. Select OK. The report is generated. On the generated report, select . The Save Snapshot window opens. In Save As, enter the name of the report, and select OK. The added report appears under Report Snapshots on the Cohorts tab. Memorized Reports in NextGen Care® Population Management Hub

Reports can be created in NextGen® Enterprise PM using specific filters and qualifying criteria to identify a specific patient population, and then saved as a memorized report. These memorized reports can be accessed in NextGen® Enterprise EHR from the Population Management Hub > Cohorts tab and used to generate a patient list for outreach purposes, such as sending messages via NextGen Care® Outreach or NextGen® PxP Portal. Cohorts

You can access the list of saved NextGen Care® Population Management Hub reports on the Cohorts tab. The following reports are available: NextGen® Enterprise EHR memorized reports. NextGen® Enterprise PM memorized reports. NextGen® Enterprise EHR and NextGen® Enterprise PM report snapshots. You can sort the data available in Cohorts by selecting on the column header. You can view the patient clinical information in Care Opportunities, only if you have permission to access patient's data. Note: In NextGen Care® Population Management Hub, only patient level NextGen® Enterprise PM reports are available. The NextGen Care® Population Management Hub reports displays a list of the individual patients identified as part of a predefined patient population. Configure User Settings

On the User Settings tab, you can customize the display for Cohorts, patient grid, Chart, and Care Opportunities. You can select the following: The report type to be displayed on the tab. The financial information columns for the patient grid. The templates that appear in the Chart list. The care opportunity layout for the Care Opportunities tab. Open NextGen Care® Population Management Hub. Select the settings icon in the lower-right corner. The Settings window opens. To configure the reports to be displayed on the Cohorts tab, select Reports on User Settings and select the desired reports. To configure the patient details to be displayed in the patient grid, do the following: Select Grid + Data. In the Cohorts section, select Limit Snapshots to last to limit snapshots to a specified number of days and enter the number of days to which the snapshots must be limited. Note: The snapshots are limited based on the date when the snapshot is created. The Limit Snapshots to last option limit
