Patient Grid

The patient grid displays the patient demographic details, financial and next appointment details. When you run the NextGen Care® Population Management Hub report, patient information appears in the patient grid. The patient grid displays the patient demographic details, patient's risk level (color-coded), patient portal enrollment status, financial information, and the next appointment details. Note: If multiple appointments are scheduled for a patient on the same day, the next immediate appointment of that day appears in the Next Appointment field when you run the report. When reports are filtered based on appointment schedule in the Appointment Book tab, the patient grid displays additional columns. View a Patient's Lab Results

You can view the patient's lab results on the Care Opportunities tab. You can view the lab results along with the date and time lab tests were performed for the selected patient. In NextGen Care® Population Management Hub, do one of the following: From the Cohorts tab, run a report. From the My Lists tab, run a list. From the Appointment Book tab, filter the patients. The patient list appears in the patient grid. In the patient grid, select the check box next to the patient. The patient’s lab result and time appears in Labs on the Care Opportunities tab. The recent lab results appear first in the list and the lab results are ordered by date the lab tests are performed on the selected patient. View a Patient's Vital Information

You can view the patient's vital information on the Care Opportunities tab. In NextGen Care® Population Management Hub, do one of the following: From the Cohorts tab, run a report. From the My Lists tab, run a list. From the Appointment Book tab, filter the patients. The patient list appears in the patient grid. In the patient grid, select the check box next to the patient. The patient’s recent vital information appears in Vitals on the Care Opportunities tab. Note: Depending on the age of the selected patient, you may view a different set of vitals. Care Opportunities Fields

You can view the field description of the appointment and clinical details on the Care Opportunities tab. The following table provides definition of the fields in appointment details and clinical details of the Care Opportunities tab. Fields Description Last Encounter Displays the date, location, and rendering provider of the last clinical and billable encounter in the patient's chart. Encounter may be locked or unlocked. Race Displays the race of the selected patient. Ethnicity Displays the ethnicity the selected patient belongs to. Current Gender Displays the current Gender for each person/patient. Sexual Preference Displays the sexual preference of the selected patient. Preferred Pronoun Displays the preferred pronoun for the selected patient. Gender Identity Displays the gender identity for each person/patient. Next Appointment Displays the selected patient's next appointment and the total number of future appointments. You can view risk score information from NextGen Population Health Solutions from NextGen Care® Population Management Hub. The Insights Ribbon icons are available only when you configure the NextGen Population Health Solutions settings in File Maintenance.
