You can view risk score information from NextGen Population Health Solutions from NextGen Care® Population Management Hub.
The Insights Ribbon icons are available only when you configure the NextGen Population Health Solutions settings in File Maintenance.

Note: The preferred browser to view Population Health Sync patient profile is Google™ Chrome
- In NextGen Care® Population Management Hub, do one of the following:
- From the Cohorts tab, run a report.
- From the My Lists tab, run a list.
- From the Appointment Book tab, filter the patients.
The patient list appears in the patient grid.
- In the patient grid, select the check box next to the patient name.
The risk score information appears in Insights Ribbon on the Care Opportunities tab.
Note: If the patient profile does not exist in NextGen Population Health Solutions, a message appears indicating that the patient information is not available when you point to the icon.
- To view more information about risk score, in Insights Ribbon, point to the circular
icon.
- To view risk score information in NextGen Population Health Solutions, in Insights Ribbon, select the circular
icon.
The Risk Report tab opens.
Note: If you are using NextGen® Enterprise 5.9 or earlier, NextGen Care® Population Management Hub shows the patient profile on your default browser.
The risk report contains three main sections:
- The Risk Overview displays the same risk scores shown when you hover over the Risk Score icon in the Patient Information Card. For definitions of each risk type, select the information
icon.
- The ACG Patient Report Overview provides the patient's Adjusted Clinical Groups (ACG) risk assessment, cost predictions, complexity of conditions, likelihood of hospitalization, utilization, and care coordination. For more information, select the information
icon for each heading.
- The Pharmacy Adherence of Select Conditions provides information on the patient's prescriptions. For more information, select the information
icon or question mark
icon for each heading.
- Scroll down to see all the sections.
You can select other tabs to review them, but you cannot take any other actions in the Patient Profile.
