You can view the communication history for the selected patients on the Outreach Log tab.
The Outreach Log tab consists of the following:

You can update and view the communication history of the selected patients on the Outreach Log. In NextGen Care® Population Management Hub, do one of the following: From the Cohorts tab, run a report. From the My Lists tab, run a list. From the Appointment Book tab, filter the patients. The patient list appears in the patient grid. In the patient grid, select the check box next to the patient name. Create one of the following: Alert Task Recall Referral Document Patient Education Message using NextGen® PxP Portal Message using NextGen Care® Outreach A log is created for the selected patients on the Outreach Log tab. Select Outreach Log. The log information for each action appears. The Documents action type in the Outreach Log tab displays the source name that is the report you used to generate the patient list from which you select the patient. The Alerts action type in the Outreach Log tab displays the date and time of the alert created, alert type, and source name. The Patient PortalUpdate the Outreach Log

The NextGen Care® Population Management Hub reports displays a list of the individual patients identified as part of a predefined patient population. Cohorts Filter My Lists Schedule You can filter the patients based on the NextGen® Enterprise PM schedule data to support care management work-flows such as care team huddles and pre-visit planning. The Referral History panel lists all the referrals that have been issued for the current patient. If necessary, you can add a new referral or update an existing referral from this panel. You can view the patient's vital information on the Care Opportunities tab. In NextGen Care® Population Management Hub, do one of the following: From the Cohorts tab, run a report. From the My Lists tab, run a list. From the Appointment Book tab, filter the patients. The patient list appears in the patient grid. In the patient grid, select the check box next to the patient. The patient’s recent vital information appears in Vitals on the Care Opportunities tab. Note: Depending on the age of the selected patient, you may view a different set of vitals. You can view the patient's lab results on the Care Opportunities tab. You can view the lab results along with the date and time lab tests were performed for the selected patient. In NextGen Care® Population Management Hub, do one of the following: From the Cohorts tab, run a report. From the My Lists tab, run a list. From the Appointment Book tab, filter the patients. The patient list appears in the patient grid. In the patient grid, select the check box next to the patient. The patient’s lab result and time appears in Labs on the Care Opportunities tab. The recent lab results appear first in the list and the lab results are ordered by date the lab tests are performed on the selected patient. When you export a graph from a patient chart to the patient NextGen® PxP Portal account, the system creates an audit trail of when it was sent and any comments which were added. The system also records when the patient opens the document from the NextGen® PxP Portal web site. Locate the graph to send under the encounter on the Encounter History tab. Select the button. Select the item to view, and select the button. Select Patient Portal History Details. The Patient Portal Document History window opens.
