You can view the communication history for the selected patients on the Outreach Log tab. Update the Outreach Log

You can update and view the communication history of the selected patients on the Outreach Log. In NextGen Care® Population Management Hub, do one of the following: From the Cohorts tab, run a report. From the My Lists tab, run a list. From the Appointment Book tab, filter the patients. The patient list appears in the patient grid. In the patient grid, select the check box next to the patient name. Create one of the following: Alert Task Recall Referral Document Patient Education Message using NextGen® PxP Portal Message using NextGen Care® Outreach A log is created for the selected patients on the Outreach Log tab. Select Outreach Log. The log information for each action appears. The Documents action type in the Outreach Log tab displays the source name that is the report you used to generate the patient list from which you select the patient. The Alerts action type in the Outreach Log tab displays the date and time of the alert created, alert type, and source name. The Patient Portal

The system records when the patient opens the document from the NextGen® PxP Portal website. Look up an enrolled patient and open the patient's record. On the Patient History tab, right-click the document name, and then select Patient Portal History Details. The Patient Portal Document History window opens. Select Close. Referral History Panel

The Referral History panel lists all the referrals that have been issued for the current patient. If necessary, you can add a new referral or update an existing referral from this panel.
