Update the Outreach Log

You can update and view the communication history of the selected patients on the Outreach Log.

  1. In NextGen Care® Population Management Hub, do one of the following:
    • From the Cohorts tab, run a report.
    • From the My Lists tab, run a list.
    • From the Appointment Book tab, filter the patients.
    The patient list appears in the patient grid.
  2. In the patient grid, select the check box next to the patient name.
  3. Create one of the following:
    • Alert
    • Task
    • Recall
    • Referral
    • Document
    • Patient Education
    • Message using NextGen® PxP Portal
    • Message using NextGen Care® Outreach

    A log is created for the selected patients on the Outreach Log tab.

  4. Select Outreach Log.

    The log information for each action appears.

    • The Documents action type in the Outreach Log tab displays the source name that is the report you used to generate the patient list from which you select the patient.
    • The Alerts action type in the Outreach Log tab displays the date and time of the alert created, alert type, and source name.
    • The Patient Portal action type in the Outreach Logtab displays the date and time of the message sent, priority, from, subject, text message, received, and source name.
    • The Population Health action type in the Outreach Log tab displays the date and time of the message sent, method used, report, result status, and source name.
    • The Patient Education action type in the Outreach Log tab displays the date and time of the message sent,document name, name of the user, and source name.
