Nursing Care Plan History Panel for the Care Coordination Home Page

The Nursing Care Plan History panel lists all the nursing care plans created for a patient. Add Referrals

You can document a referral to a specialty, obstetrics, and therapy for a patient in the Referrals Order template. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. To open the Referrals Order template, do one of the following: On the Intake template, in the Orders section, select Referrals from the list, then select Add. On the SOAP template, in the Assessment/Plan section, select Referrals from the list. On the Care Coordination Home template, in the Referral History section, select Add. Select the Referrals tab. The Referrals Order template opens. You can select the information icon to learn more about the Referrals Order template. On the Referrals tab, select the insurance provider. Enter the insurance policy number, and then select the referral type. Select the diagnosis from the list. Note: You must select at least one diagnosis for the referral. Update a Referral

Open NextGen® Enterprise EHR. Select to highlight the referral on the Referral History grid that you want to update. Select Edit.The Referrals – Orders template appears. Referrals Order Update the template with the new information. Select Save.You are returned to the Care Coordination Home Page with the entry for the referral updated in the Referral History grid. Remove a Referral

Open NextGen® Enterprise EHR. Select to highlight the referral on the Referral History panel that you want to remove. Select Remove.You are prompted to confirm the delete. Generate a Referral Document

Open NextGen® Enterprise EHR. Select to highlight the referral on the Referral History grid for which you need the document. Select Referral.The system generates and displays the requested referral document. Parent topic: Referral History Panel

Document Details of Referrals

You can document the details of referrals to order them for a patient. Open the Home or any other applicable template, select Orders from the upper navigation bar, and then select Single Order. Expand the Referrals panel. Optional: To indicate that the referral is internal, select Internal referral. Select the date of injury and insurance name. Note: By default, the encounter date appears in Date. You can change the date, if needed. The policy number is automatically filled based on the selected insurance name. In the To section, do one of the following to select the recipient: To add a referral for a specialty or provider, select Specialty/specialist name/site, and then select the specialty, provider name, and reason. To add a referral for therapy, select Therapies/Rehabilitation, and then select the therapy type and reason. Select the services requested and time frame for referral.
